Doctor Salaries in Arkhangelsk: ₽102,000 Rural Starting Pay and the Hidden 60% Staffing Clause

Part 1: What the Ministry of Health Told Me
On August 21, 2025, the Ministry of Health of the Arkhangelsk Region responded to my formal inquiry (Ref. No. 01-OP-13/1730) about contract training conditions at the Northern State Medical University (SSMU).
Question 1: Zemsky Doctor / Zemsky Feldsher
Ministry’s response: The program operates under regional Decree No. 462-pp (October 12, 2012) with no expiration date. Contract students (целевики) become eligible for the payment only when joining a facility where current staffing falls below 60%.
What this means: The 60% staffing threshold is the most consequential clause in this program. If the hospital fills its roster during your years of study, you lose the lump-sum payout entirely — regardless of any other conditions you meet.
Question 2: Settling-in Bonuses
Ministry’s response: Physicians receive a settling-in bonus (подъёмные) of ₽500,000 (~$5,000), disbursed over three years: ₽200,000 in year one, ₽100,000 in year two, and ₽200,000 in year three. Nursing staff receive ₽300,000 (~$3,000) on a different schedule: ₽100,000 in year one, ₽50,000 in year two, and ₽150,000 in year three.
What this means: These are regional payments, separate from the federal Zemsky Doctor program. The three-year disbursement structure means you cannot access the full amount at once.
Question 3: Base Salary
Ministry’s response: Salaries are governed by Decree No. 600-pp (December 25, 2012). Minimum base salaries for the «Physicians and Pharmacists» qualification group range from ₽20,355 at Level 1 to ₽23,563 at Level 4.
What this means: These are legal floors, not typical earnings. The actual take-home figure depends on northern multipliers, which the Ministry acknowledged but did not calculate.
Question 4: Real Income
Ministry’s response: No specific combined figures were provided. The Ministry cited location (Far North districts vs. territories equated to the Far North), volume of care delivered, and facility tier as variables — but offered no worked examples.
What this means: The Ministry did confirm one concrete detail: physicians under age 35 receive the full northern bonus from their first day of employment, with no seniority waiting period. That is the key income advantage for new graduates in this region.
Question 5: Housing Programs
Ministry’s response: Three tracks are available. Rental compensation reaches ₽20,000/month in cities and ₽10,000/month in rural districts. Medical workers may also receive service housing (служебное жильё). Subsidized mortgage access is provided under Regional Law No. 416-21-OZ (October 31, 2007).
What this means: ₽20,000/month for rental compensation is among the highest figures for this type of regional program in Russia.
Question 6: Internship Support
Ministry’s response: Travel and accommodation during clinical placements are covered only if these conditions are explicitly written into the individual contract training agreement (целевой договор).
What this means: No default guarantee exists. Negotiate this clause in writing before signing.
Question 7: Choosing a Workplace
Ministry’s response: Contract training follows Government Decree No. 555 (April 27, 2024). The sponsoring organization (заказчик) is the regional Ministry, with the obligatory placement territory defined as «Arkhangelsk Oblast» in the contract text.
What this means: When the sponsoring organization is the regional ministry rather than a named hospital, the specific facility is assigned at the ministry’s discretion after graduation. You cannot know at signing where exactly you will work.
Question 8: Contract Terms and Termination
Ministry’s response: The Ministry cited Government Decree No. 555 for termination procedures but provided no details on the process for transferring between facilities within the region.
What this means: Federal law does offer penalty-free exit grounds, but the Ministry gave no regional guidance on transfers. The full federal rules apply by default.
Part 2: What I Found Through Independent Research
Zemsky Doctor Eligibility and Payment Amounts
The Arkhangelsk Region has a split territorial status under Government Decree No. 1946, and that split determines your payment bracket.
Severodvinsk, Mezen District, Leshukonsky District, Pinezhsky District, and the Solovetsky Islands are classified as Far North districts. Arkhangelsk city, Kotlas, and Koryazhma carry the status of territories equated to the Far North. Both categories qualify for the same federal rates.

The 60% staffing filter creates a structural risk that does not disappear once you sign. Before committing, request an official staffing certificate (справка об укомплектованности) for the specific facility. A hospital at 55% today can reach 61% before you graduate — and your eligibility disappears with it.
The SSP the Ministry Did Not Mention
The regional Ministry did not mention the federal Special Social Payment (SSP — Special Social Payment), established under Government Decree No. 2568. For primary care physicians, this monthly supplement is one of the largest single components of rural income.

SSP applies to primary care physicians — general practitioners, pediatricians, and FAP feldshers. Narrow specialists in inpatient facilities do not receive it. A physician posted to a district center with fewer than 50,000 residents earns ₽50,000/month more than a colleague in Arkhangelsk city from this supplement alone.
Base Salary: Exact 2025 Figures
The current edition of Decree No. 600-pp (as amended September 18, 2025) sets the absolute salary floor. For recent graduates completing ordinatura, Level 2 is the applicable starting point.

Real Income — The Northern Multiplier Effect
No physician in Arkhangelsk Region receives the base salary on its own. The regional multiplier and northern bonus are applied to every ruble of base pay, and for workers under 35 both are granted at the full rate from day one — without the years of seniority normally required to reach the maximum percentage.
The calculation for a starting GP at Level 2 (base: ₽22,227):

SSP is tax-free and added on top of the after-tax base.
Adding federal and regional supplements changes the picture substantially.
Doctor in Arkhangelsk city. After-tax base: approximately ₽32,874. SSP does not apply to cities over 100,000 residents. Starting income depends heavily on overtime, night shifts, and facility-level incentive payments.
Doctor in Mezen District (Far North, rural). After-tax base: approximately ₽42,543. Federal SSP: ₽50,000/month. Regional remote-area stipend: ₽10,000/month. Combined monthly income: approximately ₽102,543 (~$1,025) on a single rate, without extra duties.
The income gap between a rural Far North physician and a city-based colleague exceeds three-to-one on a single-rate after-tax calculation (₽102,543 vs. ₽32,874).
Housing Programs
The rental compensation ceiling of ₽20,000/month in cities sits at the upper end nationally. How far it stretches depends on location.

Service housing is available for medical workers placed in both urban and rural facilities. Subsidized mortgage access is provided under Regional Law No. 416-21-OZ.
Internship Costs
Because the Ministry tied placement support to individual contract terms rather than guaranteeing it as a policy, students traveling from Moscow or St. Petersburg should budget for these costs independently.

Over six years of study with multiple mandatory placements, total out-of-pocket costs can exceed ₽100,000.
Choosing a Workplace
Under the current federal procedure (Government Decree No. 555, effective May 1, 2024), applications are submitted through the «Work in Russia» portal (trudvsem.ru). The Ministry confirmed it acts as the sponsoring organization for facilities under its jurisdiction, with placement defined as «Arkhangelsk Oblast» as a whole.
Applicants who want geographic certainty should look for contract offers from individual named hospitals. These fix both the facility and the location in the contract text from the moment of signing, removing the ambiguity of ministry-level placement.
Contract Terms and Termination
Since July 2024, Zemsky Doctor participants may transfer to a different facility within the region once, without repaying the lump sum, provided the new position is a comparable rural role (Government Decree No. 954).
Federal law (Government Decree No. 555) allows penalty-free termination on several grounds. A contract student assigned a Group I or II disability may exit without financial penalties. The same applies if an immediate family member receives a Group I disability and requires personal care. A mandatory transfer of a military spouse to a new posting is another recognized ground. These examples do not exhaust the full list in the decree — review it in full before signing.
Pros and Cons
Contract training in Arkhangelsk Region combines some of Russia’s most favorable income multipliers with a geographic structure that creates sharp income disparities between city and rural placements.
The program’s strengths are concentrated in the northern districts. A young physician posted to Mezen, Leshukonskoe, or a comparable Far North location starts at roughly ₽102,543/month (~$1,025) after tax — a combination of northern multiplier applied immediately from day one without a seniority waiting period, the ₽50,000/month SSP, and the regional remote-area stipend. The settling-in bonus adds ₽500,000 (~$5,000) disbursed over three years on a front-loaded schedule (₽200,000 / ₽100,000 / ₽200,000). Rental compensation at up to ₽20,000/month is high by national standards and covers most or all of actual market rent outside Arkhangelsk city.
The drawbacks are equally concrete. The Zemsky Doctor payout of ₽2,000,000 (~$20,000) requires that the receiving facility be below 60% staffed at the time of employment — a condition that can evaporate during your years of study. Physicians placed in Arkhangelsk city take home roughly ₽32,874/month after tax on a single rate — less than a third of their rural counterparts’ combined income — with no SSP, a lower northern multiplier, and high dependence on overtime. Clinical placement costs are not guaranteed and can exceed ₽100,000 over the course of training. Contracts signed with the regional ministry as sponsoring organization leave the specific workplace undefined until graduation.
The decision to sign calls for deliberate preparation: request the staffing certificate for your target facility, negotiate internship reimbursement in writing, and verify which sponsoring organization appears by name in the contract.
