Kherson Region for Contract Medical Students: ₽2 Million Zemsky Doctor, 2% Mortgage, and ₽80,000+ Starting Pay
The Kherson Region offers prospective physicians entering contract training (целевое обучение) one of the most generous federal support packages in Russia: a one-time Zemsky Doctor payment of ₽2,000,000 (~$20,000) and access to a mortgage at 2% annually. The figures compare favorably against most other regions.
This article is part of the Navigator for Contract Students project — an independent, region-by-region investigation of contract training conditions across Russia. For the Kherson Region, I applied the same eight-question framework used throughout the project: Zemsky Doctor eligibility, settling-in bonuses, base salaries, real income with SSP, housing programs, internship costs, workplace selection, and contract modification rules.

My Own Research
1. Support Programs: Zemsky Doctor for ₽2 Million
The primary financial support available to physicians relocating to the Kherson Region is the federal Zemsky Doctor program, extended through 2030. For Russia’s new regions, including the Kherson Region, the program sets the maximum one-time compensatory payment: ₽2,000,000 for physicians moving to rural areas, workers’ settlements, or towns with a population under 50,000. For mid-level personnel — feldshers (фельдшеры), midwives, and nurses — the payment is ₽1,000,000 (~$10,000).
Standard eligibility conditions include Russian citizenship, a completed higher or secondary medical education, and an employment contract with a state medical organization for a minimum of five years.
The most pressing question for a graduating contract student (целевик) is whether Zemsky Doctor can be accessed immediately after finishing the degree. Federal rules state that the applicant must have no outstanding financial obligations under a contract training agreement (целевой договор). In practice, this typically means completing a mandatory service period (отработка) of at least three years first — after which the five-year Zemsky Doctor contract begins. The total commitment in this scenario reaches eight years minimum.
An exception exists: the restriction is lifted when the hiring facility’s staff occupancy falls below 60%. In that case, the contract student can receive the ₽2,000,000 payment at the start of their career and serve both obligations simultaneously. Whether this applies depends on the official staffing data at the specific hospital. Applicants should ask the sponsoring organization (заказчик) about this directly, in writing, before signing.
2. Settling-In Bonus: What Exists Instead
No regional one-time settling-in bonus (подъёмные) for healthcare workers was found in open sources for the Kherson Region. What does exist, and carries considerably more long-term weight, is the federal SSP (Special Social Payment / специальная социальная выплата), introduced under Government Decree No. 2568 and substantially increased from 2024 onward.
SSP is a monthly supplement paid on top of the base salary. It applies to primary care physicians: district GPs, pediatricians, and FAP feldshers (фельдшеры). Narrow specialists working in inpatient facilities are not eligible.

A recurring monthly supplement rather than a one-time payment creates more durable income stability. For a young physician, it means a materially higher baseline from month one.
3. Base Salary
Salaries for public-sector workers in the Kherson Region, including medical staff, are governed by Decree of the Government of the Kherson Region No. 18/1-pp of March 24, 2023, «On Remuneration of Employees of State and Municipal Institutions of the Kherson Region.» The decree establishes a Uniform Tariff Schedule (ЕТС) that determines pay for all budget-funded positions.
The calculation works as follows: the base rate for Grade 1 of the schedule is set at ₽14,620 (~$146) per month as of January 1, 2023. Each position is assigned a grade (1 through 25), and each grade carries a corresponding inter-grade coefficient. The official salary for a given position equals the base rate multiplied by that coefficient.
Under the decree’s appendix, the relevant grades are: Grade 16 for general practitioners and pediatricians; Grade 17 for surgeons of all types; Grade 12 for physician-interns.

The guaranteed minimum base salary for a GP or pediatrician is ₽31,318.
4. Real Income: What a Young Doctor Actually Earns
To estimate realistic starting pay, I reviewed live vacancies on official portals. The «Work in Russia» portal (trudvsem.ru) lists a district GP vacancy at Chaplynka District Hospital — an urban-type settlement with a population under 50,000 — with a stated salary from ₽31,316.
Taken alone, that figure is modest. The SSP changes the picture. Chaplynka falls in the under-50,000 category, so the physician in this role is entitled to the maximum monthly supplement of ₽50,000.

The combined pre-tax income for a starting district GP exceeds ₽80,000 per month. Specialist salaries run higher: a dentist vacancy in Skadovsk lists ₽62,000–90,000, likely before SSP is factored in. When reviewing any contract offer, applicants should ask whether the stated figure already includes all supplements or whether SSP is paid on top.
5. Housing: 2% Mortgage as the Primary Support Tool
The Kherson Region’s main housing instrument is a federal preferential mortgage program created specifically for Russia’s new regions. It allows purchase of primary-market housing (new apartments or developer-built houses) at up to 2% annual interest, with a maximum loan of ₽6,000,000 (~$60,000) and a minimum down payment of 10%.
The down payment can be covered using a State Housing Certificate (государственный жилищный сертификат / ГЖС) available to Kherson Region residents. The certificate amount depends on family size: from ₽2,750,000 (~$27,500) for a single person to ₽6,000,000 for a family of four in 2025.
No regional programs providing service housing (служебное жильё) or rental compensation for healthcare workers were found in open sources. Before purchasing property, a relocating physician will need to cover rent independently.

A 2% mortgage creates a genuine long-term incentive to settle. It also requires committing to 20–30 years of financial obligations. The absence of any rental support adds real pressure in the months before permanent housing is secured.
6. Practical Training: The Hidden Cost of Studying Away
Contract students from the Kherson Region are enrolled at universities in other regions. In 2025, placements are offered at V.I. Vernadsky Crimean Federal University in Simferopol and at Saint Luke Lugansk State Medical University in Lugansk. The student studies in one city; practical training likely takes place at Kherson Region facilities named in the contract.
No evidence in open sources indicates that the sponsoring organization compensates students for travel or accommodation during these placements. The costs fall entirely on the student and represent a «hidden cost» of the contract.

Medical students complete several mandatory practical placements over six years. Cumulative costs across the full study period can approach ₽100,000 (~$1,000). This expense rarely appears in contract materials and must be planned for separately.
7. Choosing a Workplace: How the New Federal Procedure Works
Since May 1, 2024, contract training agreements are concluded through a new federal procedure designed to increase transparency. Applicants no longer select a region in the abstract — they choose a specific offer from a named sponsoring organization, posted on the «Work in Russia» portal (trudvsem.ru).
When reviewing Kherson Region offers, pay close attention to who is listed as the sponsoring organization (заказчик). Two configurations exist.
In the first, the sponsoring organization is a specific medical institution — for example, the State Budgetary Healthcare Institution «Henichesk Central District Hospital» (CRH). The future workplace is known from day one. The applicant knows the city, the hospital, and the department. This is the more predictable option.
In the second, the sponsoring organization is the Ministry of Health of the Kherson Region itself. The specific hospital and location are determined later, typically in the final year of study during a state assignment (распределение) process. The graduate can be placed at any subordinate facility where vacancies exist. This carries a genuine element of uncertainty.
The new portal-based system gives applicants better information than the previous process provided. It still requires careful reading of each individual offer before signing.
8. The Contract: Changing Your Workplace and Terminating Without a Fine
Legal aspects of contract training agreements, including modification and termination, are governed at the federal level by Government Decree No. 555 of April 27, 2024.
Changing hospitals. No regional regulations detailing the internal transfer procedure within the Kherson Region were found in open sources. Standard federal practice requires agreement from all parties: the employee, the current employer, the new employer, and the sponsoring organization.
Termination without financial penalties. Federal law protects contract students in several defined circumstances. A contract may be terminated without repaying the cost of education if the student or a close relative (spouse, parent, or child) is assigned a Group I or Group II disability that prevents study or work; if the student must provide full-time care for a Group I disabled close relative; if the student is the sole parent of three or more children; or if the student’s spouse is a non-conscript military service member reassigned to a different city.
Uniform federal rules apply equally to all regions, which limits the exposure from any gaps in local Kherson Region regulatory frameworks.
Pros and Cons
The Kherson Region’s contract training package rests almost entirely on federal programs — and at this moment, those programs are generous by any standard.
The financial case for relocating here is concrete: a district GP can expect combined income above ₽80,000 per month from day one, with the potential ₽2,000,000 Zemsky Doctor payment available under the 60% staffing-threshold exception. The 2% mortgage program offers a path to homeownership on terms unavailable in any other Russian region, and the State Housing Certificate can fund the down payment entirely. Government Decree No. 555 standardizes termination rights across all regions, reducing legal risk from local regulatory gaps.
The downsides are structural. The geographic distance between the training universities (Simferopol and Lugansk) and the Kherson Region creates uncompensated travel and accommodation costs during practical placements — expenses that can accumulate to ₽100,000 over six years of study with no reimbursement mechanism in place. If the sponsoring organization listed on the contract is the regional Ministry of Health rather than a named hospital, the actual workplace remains unknown at signing. And the entire financial package — Zemsky Doctor, SSP, and the 2% mortgage — depends on the continuity of current federal policy. Any future modification or termination of these programs would directly affect the value of the contract signed today.
The decision to sign requires weighing a high and genuinely attractive upside against a long-term commitment, geographic challenges during training, and full dependence on federal program stability.
