Contract Training in the Nenets Autonomous Okrug: ₽83,000 Starting Income and a ₽2M Bonus With One Condition
This article is part of the Navigator for Contract Students project — a systematic investigation of contract training agreements across Russia’s 85 regions. For the Nenets Autonomous Okrug, we apply the same eight-question framework used in every regional study: Zemsky Doctor eligibility, financial incentives, real salaries, housing programs, internship costs, workplace selection, and contract modification rules.

Part 1: What the Ministry of Health Told Me
The Department of Healthcare, Labor, and Social Protection of the Nenets Autonomous Okrug (hereinafter — the Department) responded to the project’s official inquiry on August 25, 2025, No. 01-16/233. The letter cited specific regulatory acts and payment amounts, which allowed for direct verification against open sources.
Question 1: Zemsky Doctor Program
Ministry’s response: The Department confirmed that contract students (целевики) may receive one-time compensatory payments under two parallel instruments — the federal Zemsky Doctor program (Decree of the NAO Administration No. 49-p of February 27, 2019) and the regional analog (Decree No. 91-p of April 8, 2019). Both documents set the same access condition: the employing medical facility must have a staff vacancy rate below 60%.
What this means: The payment is not automatic. A contract student signs with a region, not with a specific vacancy rate figure. Whether the threshold applies at the time of employment — several years down the line — is impossible to verify today. The Department itself acknowledged it can amend the contract training agreement (целевой договор) to substitute the assigned hospital if necessary.
Question 2: Settling-in Bonus
Ministry’s response: The Department does not use the term «settling-in bonus» (подъёмные). Instead, it references a relocation support package established by Decree of the NAO Administration No. 99-p of May 22, 2006. The package includes a one-time payment of two base salaries for the employee and half a base salary per family member, full reimbursement of travel costs within the Russian Federation, luggage compensation up to ₽50,000 (~$500) for up to five tons, and seven paid calendar days to settle in.
What this means: No single lump-sum cash payment, but the package addresses the actual logistics of relocating to the Far North — transport, family travel, time off. For a family of three, the total value can meaningfully exceed a standard one-time bonus.
Question 3: Base Salary
Ministry’s response: The Department did not state the exact salary figure, referring instead to Order No. 99 of September 30, 2016 of the Department of Healthcare, which governs the wage structure for regional state healthcare institutions.
What this means: The base salary is set by an internal regulatory document. The actual figures were located in Appendix 1 to Order No. 99 and are analyzed in Part 2 below.
Question 4: Real Income
Ministry’s response: The Department provided average physician salaries for 2024 across its two main facilities. At the State Budgetary Healthcare Institution «Nenets Okrug Hospital» (city of Naryan-Mar), the average came to ₽188,758.80/month. At the State Budgetary Healthcare Institution «Central District Polyclinic of the Zapolyarny District» (worker settlements and villages), it reached ₽238,336.30/month.
What this means: These are average figures across all physicians including those with seniority bonuses, extra shifts, and specialist pay increments. The starting income for a newly qualified doctor will be lower, but remains among the highest entry-level physician incomes in Russia.
Question 5: Housing
Ministry’s response: The Department listed two instruments. First, service housing (служебное жильё) under NAO Law No. 90-oz of July 4, 2007 — available to medical workers employed at least at a full rate in state healthcare facilities and without permanent housing in Naryan-Mar or the worker settlement of Iskatelei. Second, if service housing is unavailable, the institution pays a rent compensation of ₽15,000/month for a one-person or two-person household and ₽20,000/month for a family of three or more. A discrepancy exists: the appendix to the letter references a separate Decree No. 234-p, which cites lower figures of ₽10,000 and ₽15,000 respectively.
What this means: Ask the specific employing institution which decree currently applies before signing.
Question 6: Internship Travel
Ministry’s response: Under Appendix 4 to Decree No. 147-p of May 24, 2019, contract students receive annual reimbursement for travel from their place of study to their NAO internship site and back. The reimbursement cap is ₽20,000 (~$200) per year.
What this means: This is a rare benefit. Most regions offer nothing for internship travel. The ₽20,000 limit is sufficient for a round-trip flight from Moscow to Naryan-Mar booked in advance.
Question 7: Choosing a Workplace
Ministry’s response: Under Government Decree No. 555 of April 27, 2024, the Department posts contract training offers on the «Work in Russia» portal (trudvsem.ru). The applicant selects a specific offer — which includes the sponsoring organization (заказчик), the educational institution, and the future employer.
What this means: The key variable is who the sponsoring organization is — a specific hospital or the Department itself. If it is the Department, the actual facility where the mandatory service period (отработка) will be served is determined only after graduation.
Question 8: Contract Modification and Termination
Ministry’s response: Changing the place of employment is possible by mutual agreement of the parties under Government Decree No. 555. The Department noted that it can amend the contract to substitute the employing organization if needed. Liability for non-performance by the student is governed by Section VII of the Decree, with the student reimbursing the sponsoring organization for all support provided.
What this means: Transfer to another hospital within the NAO requires consent from the current employer, the receiving employer, and the Department. Termination without financial penalty is possible under a limited set of federal grounds.
Part 2: Independent Research
Zemsky Doctor: ₽2M — But Read the Condition Carefully
The entire NAO is classified as a Far North territory, which means the Zemsky Doctor payment for physicians relocating to rural settlements or worker settlements is ₽2,000,000 (~$20,000). For doctors employed in Naryan-Mar itself, the payment drops to ₽1,000,000 (~$10,000).
These figures are among the highest in the country. However, access depends entirely on whether the employing facility’s vacancy rate is below 60% at the time of employment. This condition applies to both the federal program (Decree No. 49-p) and the regional program (Decree No. 91-p). Neither document states what happens if the rate is exactly 60% or if it changes between the contract signing and the start of employment.
Data on current vacancy rates for specific NAO hospitals is not publicly available. A general staffing shortage exists throughout the Russian healthcare system, but that does not guarantee any particular facility falls below the threshold. A contract student should treat the ₽2M figure as a realistic possibility rather than a certainty, and should not base the decision to sign on this payment alone.
For feldshers, the Zemsky Feldsher program pays ₽1,000,000 (~$10,000) in Far North locations — also subject to the same vacancy-rate condition.
Settling-in Bonuses and the Relocation Package
The NAO relocation package under Decree No. 99-p covers four components. The one-time cash component equals two base salaries for the employee plus half a base salary per family member — so a doctor with a base salary of ₽12,300 arriving with a spouse and one child would receive approximately ₽30,750 before tax. Travel for the entire family within Russia is reimbursed in full. Luggage is covered up to ₽50,000 for up to five tons. Seven paid days off are provided for settling in.
This structure addresses the practical reality of moving to a remote Arctic region. Airfare from central Russia, moving a household, and having time to find schools and housing all carry real costs. The package reduces those costs directly rather than providing a generic lump sum.
Base Salary: The Starting Point
Appendix 1 to Order No. 99 of September 30, 2016 establishes the minimum base salaries for state healthcare employees in the NAO. For specialist physicians at the second qualification level (the standard classification for new graduates), the figure is ₽11,500/month. For district general practitioners, district pediatricians, and general practice physicians at the third qualification level, it is ₽12,300/month.
These base salary figures are the denominator for all subsequent calculations. They look low in isolation, but the NAO compensation structure is built around multipliers applied to this base.
Real Income: How a ₽83,000 Starting Salary Is Built
The NAO applies two statutory supplements to the base salary: a regional multiplier (районный коэффициент) of 1.5 and a northern bonus (северная надбавка) of 80%. A critical feature of the NAO is that the full 80% northern bonus is paid from the first day of employment. In most other Far North and equated territories, the northern bonus accumulates over years of service — starting at 10% and reaching the maximum only after five or more years. In the NAO, a new arrival gets the ceiling rate immediately.

Note: This estimate excludes payments for hazardous conditions, night shifts, seniority increments, and other incentive payments (стимулирующие выплаты). The high regional average (₽188,000–₽238,000) reflects these additions plus salary growth tied to qualification categories and years of service.
The SSP (Special Social Payment, ССВ) component deserves attention. Naryan-Mar’s population is approximately 25,000, placing it firmly under the 50,000-resident threshold. Physicians providing primary care — GPs, district pediatricians, district internists — receive ₽50,000/month tax-free under the federal SSP program. Narrow specialists working in inpatient facilities do not qualify. For a primary care physician, the SSP alone pushes the starting monthly figure above ₽80,000 before seniority increments.
No other Far North region in this project pays the full 80% northern bonus from day one. A physician who moves to a comparable northern region typically waits four or five years before reaching that rate, receiving ₽14,760 less per month in the interim. In the NAO, that differential begins accumulating from the first paycheck.
Housing: What the Compensation Actually Covers
Service housing is the better option — it eliminates rent costs entirely. Access depends on availability of the housing fund and on the applicant not having received federal housing assistance after reaching adulthood. The employing institution processes the application.
When service housing is unavailable, the institution pays rent compensation. The main letter cites ₽15,000/month for one or two persons and ₽20,000/month for three or more. The appendix references a separate decree with lower figures (₽10,000 and ₽15,000). Until this discrepancy is resolved officially, applicants should ask the specific employer which decree governs their case.

The compensation covers between half and two-thirds of a one-bedroom apartment. A doctor will pay the remainder out of pocket. On a starting income of ₽83,000, a monthly shortfall of ₽8,000–₽13,000 is manageable but not trivial. Service housing, where available, removes this calculation entirely.
Internship Travel: A Benefit Most Regions Skip
Under Decree No. 147-p, contract students receive one annual reimbursement of up to ₽20,000 (~$200) for round-trip travel between their university and the NAO internship site. A round-trip economy flight from Moscow to Naryan-Mar typically costs ₽10,000–₽15,000 when booked in advance, and can exceed ₽25,000 closer to departure. With reasonable planning, the ₽20,000 cap covers the full fare. The reimbursement applies once per year, not once per internship visit.
Most Russian regions include no provision of this kind. Students in those regions bear the travel cost themselves — which accumulates to ₽40,000–₽100,000 over a six-year degree program.
Choosing a Workplace: Who Is the Sponsoring Organization?
Since May 1, 2024, all contract training offers must be posted on the «Work in Russia» portal (trudvsem.ru). An applicant reviews a specific offer, which names the sponsoring organization (заказчик), the educational institution, the required specialty, and the future employer.
Two distinct scenarios arise in NAO offers. When a specific hospital — say, the Nenets Okrug Hospital — is the sponsoring organization, the future workplace is known at the moment of signing. When the Department of Healthcare itself is the sponsoring organization, the actual facility is assigned after graduation. The student commits to working somewhere in the NAO without knowing which town or hospital until the state assignment (распределение) is made. Both types of offers appear on the portal; reading the details before applying determines which scenario applies.
Contract Terms: Transfers and Exit Conditions
Changing the assigned workplace within the NAO is possible by mutual agreement of all parties — the current employer, the receiving employer, and the Department. No NAO-specific regulation was found detailing the process beyond the federal framework. In practice, this means a transfer is achievable but not guaranteed on request.
Termination of the contract training agreement (целевой договор) without repaying the sponsoring organization’s costs is possible under a defined set of circumstances set out in Government Decree No. 555. A student may exit without penalty if they or an immediate family member receives a Group I or Group II disability classification; if they must provide full-time care for a parent, spouse, or child with a Group I disability; or if their spouse is a military servicemember relocating to a new duty station and employment in the new location is impossible. Outside these grounds, the student reimburses all support received.
Pros and Cons
The NAO is one of a small number of Russian regions where the compensation package for incoming physicians is both generous and well-documented. The Department’s response cited specific decrees, exact sums, and clear eligibility conditions — a standard that most regional health ministries do not meet.
The case for choosing the NAO rests on four concrete factors. The immediate 80% northern bonus is the most financially material: it adds roughly ₽14,760/month to income from the first paycheck, rather than accumulating over four to five years as in most northern regions. Combined with the SSP of ₽50,000/month for primary care physicians and a regional multiplier of 1.5, the starting monthly income reaches approximately ₽83,000 (~$830) — before night shifts, seniority, or qualification increments. The average physician salary across NAO institutions in 2024 was ₽188,758 in the city and ₽238,336 in rural areas, confirming that income grows substantially with experience. The relocation package covers family travel, five tons of luggage, and seven paid settling-in days. Annual travel reimbursement up to ₽20,000 for internship trips is a benefit few other regions offer.
The case against centers on three uncertainties. The largest payment — ₽2,000,000 (~$20,000) for rural relocation — is conditional on the employing facility having a vacancy rate below 60%, a figure that cannot be verified years in advance. Rent compensation (₽15,000–₽20,000/month) covers roughly half to two-thirds of a one-bedroom apartment in Naryan-Mar, leaving a monthly gap of ₽8,000–₽13,000; service housing removes the problem but depends on fund availability. The non-financial conditions — polar nights lasting up to two months, temperatures below −30°C, and isolation from the nearest large city — are as relevant as any financial calculation.
The decision to sign a contract training agreement with the NAO should rest on the guaranteed components: the northern bonus structure, the SSP, and the relocation package. The ₽2M Zemsky Doctor payment is worth pursuing but should not be the reason you sign.
