Zaporozhye Region: ₽2M Zemsky Doctor, 2% Mortgage, and ~₽142,500 Monthly Income for Doctors

At the time of preparing this material, no official response from the Ministry of Health of the Zaporozhye Region was available. The analysis below draws on federal regulatory acts, decrees of the Government of the Zaporozhye Region, and open sources.


Question 1. The Zemsky Doctor Program

In Zaporozhye, the elevated payout coefficients that apply to the Far East and the Far North are in force for this region as well.

Legal basis: Government Decree No. 954 of July 13, 2024.

Upon employment, a physician receives ₽2,000,000 (~$20,000) and a feldsher receives ₽1,000,000 (~$10,000). Under the Tax Code, this payment is not subject to personal income tax — the full amount goes to the recipient.

The territorial coverage here differs from most Russian regions. Elsewhere, cities above 50,000 residents are routinely excluded from the program. In the new federal subjects, the list of eligible settlements is interpreted as broadly as possible because the personnel shortage is acute across the entire territory.

For contract students (целевики), one restriction applies: the right to the payout arises only after completing the mandatory service period (отработка) under the contract training agreement (целевой договор) — or upon extending the contract to five years.


Question 2. Settling-in Bonuses

Unlike resource-extraction regions such as Yamal or Sakhalin — which have regional laws providing ₽300,000–500,000 to young specialists — the Zaporozhye region has not yet established its own regulatory framework for settling-in bonuses (подъёмные). No regional lump-sum payments appear in publicly available sources.

What is available upon relocation is the federal relocation package guaranteed by the Labor Code of the Russian Federation. It covers travel tickets and luggage transport for the employee and family members, along with paid leave for settling in. This is not a cash payment; it is expense reimbursement.


Question 3. Base Salary

The remuneration system for physicians, including new graduates, is based on the Unified Tariff Scale. The base rate for the 1st category stands at ₽14,620 as of January 1, 2023, and the final salary is calculated by multiplying this rate by an inter-category coefficient.

Graduates entering employment as trainee doctors are assigned to one of two categories. Surgical specialties — surgeons, anesthesiologists-resuscitators — and general practitioners fall under the 10th category, with a coefficient of 1.397 and a calculated base salary of ₽20,424. All other medical specialties fall under the 9th category, with a coefficient of 1.328 and a base salary of ₽19,415. Experienced physicians with a professional qualification grade are rated from the 10th through 14th categories.


Question 4. Real Income

The base salary is not what a physician in Zaporozhye actually takes home. The full income is built from several layers of bonuses and federal payments.

Incentive and compensatory payments. There is no separate «young specialist» bonus in this region (unlike in education). Instead, a labor intensity bonus of up to 50% of the base salary applies, calculated based on workload and priority tasks. A specialized care bonus adds 20%. Working in rural areas adds a further 10%. Compensatory payments cover work with HIV and tuberculosis patients (60%), psychiatric and addiction medicine teams (25%), and night shifts (35–50% of the hourly rate).

Special Social Payment (SSP). Under Government Decree No. 2568, primary care physicians in settlements with fewer than 50,000 residents receive a monthly federal supplement of ₽50,000. The majority of Zaporozhye’s territory falls under this threshold. The payment is tax-free and is excluded from average-earnings calculations used for vacation pay and sick leave.

SSP applies to primary care physicians — district GPs, pediatricians, general practitioners, and FAP feldshers. Narrow specialists working in inpatient facilities do not receive SSP.


Question 5. Housing Programs

The more practical housing instrument here is not service housing — which is limited in supply and frequently requires renovation — but a preferential mortgage at 2% per annum.

The program has one feature that distinguishes it from the «mainland» Russian market: it extends to secondary housing. Federal preferential mortgage programs elsewhere in Russia typically apply only to new construction, and new builds are scarce in Zaporozhye. Here, a buyer can purchase a finished apartment on the open secondary market.

The Zemsky Doctor payout of ₽2,000,000 (~$20,000) covers the full cost of housing in most rural locations and over 50% of an apartment price in the region’s larger cities.


Question 6. Support During Internship and Practical Training

Support for contract students is governed by Decree of the Government of the Zaporozhye Region No. 417 of November 28, 2023. In January 2025, Decree No. 9 introduced wording clarifications to this document.

The regional Ministry of Health typically assigns contract students to three partner universities: Rostov State Medical University (Rostov-on-Don), Crimean Federal University (Simferopol), and Volgograd State Medical University (Volgograd). Travel to internship locations is paid by the students themselves. In standard contract training agreements, travel reimbursement is often phrased as a «right» rather than an «obligation» of the sponsoring organization (заказчик) — a wording that allows hospitals to decline payment without legal consequence.

The regional supplement to the student stipend provides partial offset. Decree No. 417 provides ₽3,000 per month for students and ₽20,000 per month for ordinatura (residency) trainees.


Question 7. Choosing a Workplace

Since May 1, 2024, contract training placements are processed through the «Work in Russia» portal (trudvsem.ru). When reviewing a contract offer, the applicant must check the «Sponsoring Organization» column carefully. If the sponsoring organization is the regional Ministry of Health rather than a named medical facility, the actual workplace will be determined by the ministry after graduation.

Given the centralized structure of healthcare management in the region, assignment to underserved rural districts — where the staffing shortage is most acute — is a realistic outcome. Placement in the comparatively stable urban environment of Melitopol cannot be assumed.


Question 8. Contract Terms and Termination

Changing the workplace within the region. The Zaporozhye regulatory framework contains no prohibition on transfers between healthcare facilities within the subject. The regional Ministry of Health generally approves internal transfers, since retaining the physician within the region is the primary objective.

Terminating the contract. Federal legislation — Government Decree No. 555 of April 27, 2024 — permits penalty-free termination of a contract training agreement in several defined circumstances. Establishing a Group I or II disability is one ground. The necessity of providing personal care for a close relative with a Group I disability is another. A third ground — the relocation of a military spouse to a new posting — is particularly relevant given the large military presence in the region. These are federal provisions and apply uniformly across all regions.


Pros and Cons

Zaporozhye combines the highest available tier of federal financial incentives with the realities of working in a region that remains under special administrative conditions.

The financial case for this region is unusually strong. The Zemsky Doctor payout of ₽2,000,000 (~$20,000) is available across essentially the entire territory, with no exclusions for larger cities, and arrives tax-free. Combined with the SSP of ₽50,000 per month and performance bonuses reaching 50% of base salary, a young district GP can realistically expect a monthly income around ₽142,500 (~$1,425) — without seniority supplements or qualification grades. The 2% mortgage program, which applies to secondary housing, is the most practical housing instrument available. At rural real estate prices, the Zemsky Doctor payout alone can cover the full purchase price of an apartment.

The risks are concrete and should be weighed carefully. The region operates under a special security regime: power and water supply interruptions occur, a curfew is in force, and movement restrictions apply. These are not temporary inconveniences. A new graduate without seniority grades or a specialist category relies entirely on the intensity bonus and SSP for income above the base rate — there is no «young specialist» supplement to bridge the gap. Internship logistics are complex: round-trip travel plus accommodation for a single four-week internship from Rostov-on-Don costs approximately ₽28,000, and contracts rarely guarantee reimbursement. Applicants who sign with the Ministry of Health rather than a named facility accept uncertainty about their eventual workplace.

The decision requires a clear-eyed assessment of personal priorities, tolerance for instability, and career timeline.
