Contract Training in Perm Region: ₽2M Zemsky Doctor, ₽2M Regional Bonus — No Internship Support, No Housing Data

To get the most complete picture of contract training (целевое обучение) conditions in Perm Region, I sent an official inquiry to the regional Ministry of Health, framing the same eight-question framework the project applies to every region. The ministry replied on August 19, 2025 (No. 34-06-01-4088-out).
The response is a textbook example of an answer that says nothing actionable. Legally correct, it leaves a prospective contract student (целевик) alone with the questions that actually matter.
Part 1: What the Ministry of Health Told Me
Question 1: Zemsky Doctor/Zemsky Feldsher
Ministry’s response: Graduates who take jobs in state healthcare institutions outside the city of Perm are entitled to participate in the Zemsky Doctor/Zemsky Feldsher and «Medical Personnel of the Prikamye» programs, which provide one-time compensation payments.
What this means: The programs exist and are accessible to contract students, but only if they work outside the regional capital. Those placed in Perm itself are excluded from both programs. The ministry gave no figures and no expiry dates.
Question 2: Settling-in Bonuses (подъёмные)
Ministry’s response: Not addressed. The letter described program names but listed no amounts.
What this means: An applicant reading only this letter would not know whether any lump-sum payment exists, let alone how much it is. The figures had to be found through independent research.
Question 3: Base Salary
Ministry’s response: Salary in each institution is governed by Perm Region Government Decree No. 1190-p of December 31, 2015. It depends on qualification category and academic degree, so providing an average figure is not possible.
What this means: The ministry redirected the question to a regulatory document rather than stating a number. The decree exists, the base rate is findable, but the ministry chose not to summarize it.
Question 4: Real Income and Guaranteed Supplements
Ministry’s response: Same deflection as Question 3. Individual salaries vary, no average is providable.
What this means: The ministry gave no data on incentive payments, compensatory payments, or what a newly graduated physician could realistically expect to earn in the first year of practice.
Question 5: Housing Programs
Ministry’s response: Depending on the municipality, support measures include service housing, partial mortgage interest compensation, and land plots for individual housing construction.
What this means: Three mechanisms are confirmed. No amounts, no conditions, no list of eligible municipalities, and no answer on whether any of these apply inside Perm itself.
Question 6: Internship Support During Studies
Ministry’s response: Travel compensation and accommodation during practical training are not provided for in the contract training agreement.
What this means: This is the one unambiguous answer in the letter. All costs incurred while traveling to and staying at a placement hospital during mandatory internships are borne entirely by the student and their family.
Question 7: Choosing a Workplace
Ministry’s response: Under Government Decree No. 555 of April 27, 2024, applicants select a specific proposal posted by a sponsoring organization (заказчик) on the «Work in Russia» portal (trudvsem.ru) and compete for that specific slot. Each proposal identifies the future employer.
What this means: The workplace is fixed before enrollment. An applicant knows which hospital they are contracting with before they submit documents to the university.
Question 8: Changing Workplace and Terminating the Contract
Ministry’s response: Not addressed.
What this means: This is a complete omission. The conditions for transferring to a different facility or exiting the contract without financial penalty were left entirely unanswered.
Part 2: What I Found Through Independent Research
Zemsky Doctor Eligibility and Payment Amounts
The federal Zemsky Doctor and Zemsky Feldsher programs are extended through 2030. A contract student who completes training and takes a position outside Perm is eligible, subject to the five-year full-time employment requirement.
Payment amounts vary by territory. Three districts in Perm Region — Gaynsky, Kosinsky, and Kochevsky — are equated to the Far North, which triggers the higher federal rate: ₽2,000,000 (~$20,000) for physicians and ₽1,000,000 (~$10,000) for feldshers. Settlements on the regional list of remote and hard-to-reach territories (approved by Perm Region Government Decree No. 615-p of August 20, 2020) carry the intermediate rate of ₽1,500,000 (~$15,000) for physicians. All other rural settlements and towns with fewer than 50,000 residents fall under the standard rate of ₽1,000,000 (~$10,000) for physicians, ₽500,000 (~$5,000) for feldshers.
The ministry’s letter did not specify any of these figures.
Settling-in Bonuses: The «Medical Personnel of the Prikamye» Program
The regional program, established by Perm Region Government Decree No. 326-p of April 20, 2022, runs parallel to the federal Zemsky Doctor. Most municipalities in the region offer ₽2,000,000 (~$20,000) to physicians who take positions there. Four cities — Solikamsk, Krasnokamsk, Kungur, and Lysva — fall under an exception rate of ₽1,000,000 (~$10,000).
Combining both programs is theoretically possible if the position qualifies under each separately. Confirm eligibility for each program with the specific facility before signing.
Special Social Payment (SSP)
The ministry’s letter did not mention SSP, a monthly federal supplement paid on top of base salary to primary care physicians working in smaller settlements. Physicians in towns under 50,000 residents receive ₽50,000/month; those in towns of 50,000–100,000 residents receive ₽29,000/month. The payment is tax-free and excluded from official average-wage statistics.
SSP applies to primary care physicians: district general practitioners (терапевты), pediatricians, and FAP feldshers. Narrow specialists working in inpatient departments do not receive it.
To illustrate combined income: a GP earning ₽80,000 gross in a town under 50,000 residents takes home approximately ₽69,600 after the 13% income tax, plus ₽50,000 SSP (tax-free), for a combined monthly income of roughly ₽119,600 (~$1,196). That is above the regional median by any benchmark.
For a GP taking a position in a rural district or a small town under 50,000 residents, SSP alone covers rent in Berezniki or Chusovoy with money left over.
Base Salary and Real Income
The salary framework under Decree No. 1190-p has three layers. The base salary (должностной оклад) is the only guaranteed portion. According to the 2018 version of the decree, a specialist physician without experience in the second qualification group received a base of ₽11,000 (a figure so low it functions as an accounting floor, not a living wage). On top of that, compensatory payments (компенсационные выплаты) cover night shifts, hazardous conditions, and rural work (25% of base salary). Incentive payments are the largest and least predictable component, tied to each hospital’s budget, KPI fulfillment, and management discretion.
Regional wage benchmarks vary sharply by source. GorodRabot.ru, drawing from active vacancies, put the average at ₽126,528 in August 2025. Gogov.ru, drawing from Rosstat figures, reported ₽83,992 for May 2025. The hh.ru median for August 2025 was ₽67,800. These gaps reflect methodology: vacancy-based averages are inflated by high-demand postings; Rosstat figures smooth out part-time arrangements common in Russian healthcare.
Against this backdrop, a starting physician salary of ₽50,000–₽100,000 looks uneven. In a well-funded outpatient clinic with a strong incentive pool, it competes with the regional median. In a hospital with a tight budget, it does not.

The spread is wide enough to make the phrase «physician salary in Perm Region» nearly meaningless without specifying the institution and the position type. A GP at Clinic No. 4 earns roughly twice what a GP at Hospital No. 3 earns, even within the same city.

After the mandatory service period (отработка), the salary trajectory looks considerably better. The five years of experience that the contract forces you through also make you substantially more marketable.
Housing Programs
Rent subsidy. The «Medical Personnel of the Prikamye» program includes partial rent compensation of up to ₽10,000 per month, but only for nursing staff (nurses and feldshers) working in Perm. Physicians are excluded from this measure.
Mortgages. No special regional mortgage product exists for physicians in Perm Region. Physicians can access general federal programs (Family Mortgage, IT Mortgage) on the same terms as any other borrower. Commercial bank mortgage rates apply for those who do not qualify for federal programs.
Service housing. Regional law allows physicians who have worked ten years in small towns or rural settlements to privatize their service apartment. The catch: as of July 2025, the entire regional specialized fund held 38 apartments. Demand far exceeds supply.
Land plots. Perm Region Law No. 111-PK of October 3, 2022 gives physicians and teachers the right to receive a free land plot for individual housing construction, subject to working in a settlement of under 10,000 residents, having at least six months of employment in the region, and having no existing real estate ownership in that municipality.
For a physician placed in Perm itself, none of the meaningful housing benefits apply. The rent subsidy covers only nursing staff, service housing is concentrated outside the capital, and land plots require a small-settlement posting. The capital has the highest rent in the region.

Internship Costs During Studies
The ministry confirmed this directly: no support. Over six years of study, students may face several mandatory placements at hospitals outside their university city. A realistic estimate for travel, short-term accommodation, and daily expenses across these placements runs to ₽100,000–₽180,000. The family absorbs this entirely.
Choosing a Workplace
Under the new procedure established by Government Decree No. 555 of April 27, 2024, the process works as follows: a sponsoring organization posts a specific job offer on the «Work in Russia» portal (trudvsem.ru), naming the hospital and the number of contract slots. An applicant selects that offer, submits a consent form, and competes against other applicants for that specific position. The contract is signed only after enrollment.
There is no general ministerial agreement that assigns you to an unspecified facility later. The workplace is known before admission.
Contract Terms and Termination
Changing workplace within the region is not provided for under Decree No. 555. The contract is concluded with a specific employer; obligations run to that employer, not to the regional ministry as an abstract entity. Transferring to a different hospital would require renegotiation that the federal framework does not anticipate.
Penalty-free termination is possible under the grounds listed in Section VII of Decree No. 555. These include the establishment of Group I or II disability affecting the student or a required employment condition, the need to care for a disabled close relative when no other person is legally obligated to do so, and several other documented life circumstances. Applicants and their families should read Section VII of Decree No. 555 in full before signing. The list of grounds is exhaustive; circumstances outside it trigger full repayment of tuition costs.
Pros and Cons
Contract training in Perm Region is a viable path to a physician career, but the conditions differ sharply depending on where in the region a graduate ends up working, and the gap between the best and worst scenarios is large enough to matter.
The financial case for rural and small-town placements is strong. Combining Zemsky Doctor (up to ₽2,000,000 in Far North-equivalent districts, ₽1,000,000 in standard rural areas) with the regional «Medical Personnel of the Prikamye» bonus (up to ₽2,000,000 in most municipalities) creates a lump-sum incentive of up to ₽4,000,000 (~$40,000) for those who qualify for both. SSP adds up to ₽50,000 per month tax-free for GPs in towns under 50,000 residents, pushing combined monthly income well above the regional median. The new procurement system (Decree No. 555) lets applicants see the specific hospital before committing, which removes the ambiguity of older ministerial contracts.
The weaknesses are concentrated in three areas. First, a physician placed in Perm receives none of the major financial incentives: no Zemsky Doctor, no regional bonus, no SSP, no rent subsidy, and no practical path to service housing. Second, the service housing stock is too thin to be a realistic planning assumption: 38 apartments statewide is a program in name more than in practice. Third, the ministry refused to provide any salary data, forcing applicants to reconstruct the picture from vacancy portals. Starting salaries range from ₽50,000 to ₽100,000+ depending on the institution (a range wide enough to constitute a genuine lottery for anyone locked into a specific hospital for three to five years). Internship costs over six years of study add a hidden expense of roughly ₽100,000–₽180,000 with no reimbursement path.
The decision to sign a contract training agreement in Perm Region is one that should be made with the specific hospital, location, and payment combination in hand, not on the strength of the ministry’s official letter, which confirms that programs exist without telling you whether they apply to your case.
