Medical Contract Training in Primorsky Krai: Up to ₽4M for Shortage Specialists, But Payouts Are Mutually Exclusive

A Note on Sources
The regional Ministry of Health sent a response to my inquiry — but it answered different questions than the ones I asked. All analysis below is therefore based on independent research.
This article draws on the Law of Primorsky Krai dated December 19, 2024, No. 694-KZ; Government Decree of Primorsky Krai dated April 24, 2025, No. 379-pp; Order of the Ministry of Health of Primorsky Krai No. 18/pr/1027; and Order No. 18/pr/1825 (as amended on November 27, 2025).
What the Legislation Covers: A Quick Map
Primorsky Krai belongs to the DFO — Far Eastern Federal District — which means the federal Zemsky Doctor program pays physicians ₽2,000,000 (~$20,000) and feldshers ₽1,000,000 (~$10,000) for relocating to settlements under 50,000 residents. This payment stacks with regional measures.
On top of that, regional law offers differentiated lump-sum settling-in bonuses (подъёмные) ranging from ₽750,000 to ₽1,000,000 depending on the district — but receiving any regional payment now excludes the others. The 2025 legislation introduced a strict «either/or» rule across three options: the settling-in bonus, the shortage specialty payment, or the mortgage subsidy.
The remuneration system is governed by Order No. 18/pr/1027. The minimum base salary for a district GP is ₽15,400, with a guaranteed regional monthly social payment of ₽10,000 during the first five years of service layered on top. Real income is calculated by multiplying the base against regional and Far Eastern multipliers, then adding the federal SSP — Special Social Payment (ССВ) — of ₽50,000/month in settlements under 50,000 residents.
On housing: a mortgage down payment subsidy of up to ₽800,000 (~$8,000) exists, capped at 10% of property value, while rent compensation has been extended through 2029 with limits up to ₽30,000/month in Vladivostok. No regulatory act mandating travel or accommodation compensation for contract students (целевики) during practical training was found.
Placement follows federal procedure, but the link to a specific municipality is now critical. Order No. 18/pr/1825 designates specific positions at named hospitals as shortage roles — only those exact positions unlock the maximum payment. The mandatory service period (отработка) for all regional payouts is five years, with proportional repayment triggered by early resignation.
My Own Investigation
1. Zemsky Doctor: The Payment That Stacks
Unlike the regional system described below, the federal Zemsky Doctor program is straightforward. Physicians relocating to settlements under 50,000 residents receive ₽2,000,000. Primorsky Krai belongs to the DFO, which qualifies the entire region for the ₽2M rate rather than the standard ₽1M applicable in rural areas outside the Far North and the Far Eastern zone.
The key advantage: this payment combines with any regional measures. The cities of Vladivostok, Artyom, Ussuriysk, and Nakhodka fall outside the program because their populations exceed the threshold.
2. The Three-Chair Dilemma: Which Regional Payout to Choose?
Starting in 2025, Law No. 694-KZ forces a hard choice. A physician can receive only one of three support measures: the settling-in bonus, the shortage specialty payment, or the mortgage subsidy. Taking the settling-in bonus and the mortgage money simultaneously is not permitted.
Here is the mathematics of each option.
The settling-in bonus under Article 3 is the most flexible instrument: money transfers to your account with no restrictions on how it is spent. Physicians in northern districts — Dalnegorsky, Kavalerovsky, Krasnoarmeysky, Lazovsky, Olginsky, Pozharsky, and Terneysky — receive ₽1,000,000. All other municipalities in the region pay ₽850,000, while Vladivostok, Artyom, Ussuriysk, and Nakhodka offer ₽750,000.
The shortage specialty payment under Article 5(3) is fixed at ₽2,000,000 — the most financially attractive scenario by a wide margin. Order No. 18/pr/1825 defines the list. The critical distinction: shortage status is tied not to a specialty in the abstract but to a specific position at a specific hospital. The 2025 approved list includes anesthesiologists-resuscitators at Pozharskaya CRH (Central District Hospital), Nakhodka City Hospital, Arsenyev City Hospital, and Khankayskaya CRH; neurologists in Nakhodka, Dalnerechensk, Shkotovskaya CRH, and Krasnoarmeyskaya CRH; cardiologists in Nakhodka and Dalnegorsk; an oncologist in Dalnegorsk; a psychiatrist at Lazovskaya CRH in the settlement of Preobrazhenie; and a district pediatrician at Lesozavodsk City Hospital, added by the November 27, 2025 amendment. A position not on this list earns no ₽2M bonus regardless of how rare the specialty appears.
The mortgage down payment subsidy under Article 5(4) offers up to ₽800,000, capped at 10% of the property value. The restrictions are tight: the mortgage must have been issued from January 1, 2025 onward, and only ready-made housing in the same municipality qualifies. For most physicians, this option makes little economic sense. Standard settling-in bonuses range from ₽750,000 to ₽1,000,000 — matching or exceeding the subsidy amount, with no restrictions on use.
3. Base Salary: ₽15,400 as the Starting Point
Order No. 18/pr/1027 sets minimum base salaries as follows. District GPs and pediatricians: ₽15,400. Inpatient and emergency physicians: ₽15,400. Surgeons and anesthesiologists-resuscitators: ₽15,970. Paramedical staff — feldshers and midwives — between ₽10,837 and ₽11,407.
These figures serve as the base for the regional multiplier (1.2–1.4) and the Far Eastern bonus (up to 30%). Under the remuneration structure, the base salary must constitute at least 55% of total pay, with incentive payments covering the remaining 45%.
4. Real Income: What the Numbers Look Like
From 2025, the income formula for a starting district GP works as follows: base salary of ₽15,400 multiplied by combined coefficients of approximately 1.5, yielding roughly ₽23,000 before incentive payments. Adding the federal SSP of ₽50,000/month (applicable in settlements under 50,000 residents) and the regional monthly supplement of ₽10,000 brings the guaranteed floor to approximately ₽83,000–₽85,000 per month. With incentive payments, a starting rural GP can expect around ₽130,000 (~$1,300) per month. An optional mentorship supplement of ₽7,000/month is available where applicable.
One caveat applies to the shortage list specialties. SSP is designed for primary care physicians: district GPs, pediatricians, and FAP feldsher staff at rural medical outposts. Narrow specialists working in inpatient settings — anesthesiologists, cardiologists, oncologists, neurologists — do not receive SSP. Their income calculation omits the ₽50,000 component.
5. Housing: Rent Compensation Through 2029
While a young specialist saves toward a home purchase, the region covers rental costs. Law No. 694-KZ extended rent compensation through the end of 2029. The limits remain among the highest by national standards: ₽30,000/month in Vladivostok and ₽20,000/month across regional districts.

6. Practical Training: The Hidden Cost
For contract students enrolled at universities in central Russia, practical training in Primorye is a financial challenge the regional regulatory framework ignores entirely. No mechanism for travel or accommodation reimbursement appears in the new legislative package.

Over the full period of study, a family budget can absorb more than ₽250,000 (~$2,500) in travel costs alone.
7. Choosing a Workplace: The Vacancy Risk
The main risk for a contract student (целевик) is misaligning with the right vacancy. A contract training agreement (целевой договор) signed with the regional Ministry of Health as sponsoring organization (заказчик) — rather than with a specific hospital — may result in state assignment (распределение) to a facility where the student’s specialty carries no shortage status. In that scenario, the contract student loses access to the ₽2,000,000 bonus and receives the standard ₽850,000 settling-in bonus instead.
To access the maximum payout, the agreement must be tied specifically to an institution named in Order No. 18/pr/1825 — for example, Nakhodka City Hospital or Dalnegorsk Central City Hospital.
8. Contract Terms and Exit Conditions
Regional legislation binds all payouts to a five-year mandatory service period. Federal Government Decree No. 555 provides parallel protections for contract students.
Penalty-free contract termination is available in four circumstances: recognition of the student or graduate as having Disability Group I or II; necessity of caring for a close relative classified as Disability Group I; relocation with a military spouse reassigned to a new duty station, a frequent situation in Primorye as a major garrison region; and recognition as the sole parent of a child with a disability.
Early voluntary resignation outside these grounds triggers repayment proportional to the unserved time. Misuse of the mortgage subsidy requires full repayment regardless of time served.
Pros and Cons
Primorsky Krai offers one of the larger combined financial packages in Russia for physicians willing to work in rural areas. The 2025 legislative structure, however, requires careful navigation before any contract is signed.
The case for signing here starts with cumulative potential. A shortage specialist combining the ₽2,000,000 (~$20,000) regional bonus with the ₽2,000,000 Zemsky Doctor payment enters the job market with ₽4,000,000 (~$40,000) in combined incentives. Rent compensation at close to market rates, extended through 2029, removes the immediate housing burden. The guaranteed ₽10,000/month regional supplement provides a financial floor during the first five years, and the settling-in bonus for those who take it rather than the shortage payment carries no usage restrictions.
The case against centers on three structural problems. The ₽2M shortage bonus is not a broad regional offer — it is attached to a short, specific list of positions at named hospitals, and missing that list means receiving ₽850,000 instead. The 2025 «either/or» rule makes the three payouts mutually exclusive: a physician cannot combine the settling-in bonus with the mortgage subsidy, which eliminates the ability to stack regional measures. The base salary of ₽15,400 is very low; take-home pay depends almost entirely on multipliers and incentive payments that hospital management controls. Practical training costs receive no compensation, creating a substantial upfront burden for students studying far from the region.
The contract is worth signing only after the specific shortage position has been confirmed in Order No. 18/pr/1825 before the agreement is finalized.
