Zemsky Doctor in the LPR 2025: ₽2,000,000 Upfront and New Salary Scales for Physicians


The Luhansk People’s Republic (LPR) currently offers some of the highest financial incentives for physicians in Russia, backed by federal subsidies and a newly structured regional salary grid.

This article is part of the Navigator for Contract Students project — an independent analysis of contract training (целевое обучение) opportunities across Russian regions. For the LPR, we apply the same eight-question framework used throughout the project: Zemsky Doctor eligibility, financial incentives, real salaries, housing programs, internship logistics, workplace assignment, and contract termination rules.

Note: As of 2025, 1 USD ≈ 100 RUB. All figures are in Russian rubles (₽) unless otherwise stated.

This analysis is based on Russian Federation regulations and LPR Government documents effective January 1, 2025 (Resolution No. 36/23 and its supplements).


1. The Zemsky Doctor Program

Due to the region’s acute personnel shortage, the administrative process for receiving payouts has been streamlined. Since 2024, payout amounts in the LPR have been doubled compared to most other Russian regions — the LPR falls under Government Decree No. 954 of July 13, 2024, which established enhanced rates for the new federal territories.

Eligibility technically requires a facility staffing level above 60%. Most LPR hospitals currently operate at 30–40% capacity, which in practice means a newly hired physician qualifies immediately upon employment.

No additional regional settling-in bonuses (подъёмные) have been found in open sources. Applicants should plan around the federal ₽2,000,000 only.


2. 2025 Salaries: The Tariff Grid Breakdown

Effective January 1, 2025, the LPR established a base rate for Grade 1 of the Unified Tariff Schedule (ЕТС) at ₽15,366. The guaranteed base salary is calculated by multiplying this rate by the applicable tariff coefficient. These are the «raw» figures that appear in employment contracts — before allowances and bonuses.

Surgical and High-Complexity Specialties

Tariff Grade 14 (coefficient 1.858) — base salary: ₽28,550.

This grade covers surgeons of all types (pediatric, vascular, thoracic, maxillofacial, plastic), neurosurgeons, oncologists, trauma-orthopedists, burn specialists, transplantologists, coloproctologists, and urologists. Anesthesiologists-resuscitators, operating obstetrician-gynecologists, endoscopists, forensic medical examiners, pathologists, and general practitioners (family doctors) also fall under Grade 14.

Therapeutic and Diagnostic Specialties

Tariff Grade 13 (coefficient 1.743) — base salary: ₽26,783.

The largest physician category. District internists and pediatricians, neonatologists, and geriatricians belong here, alongside cardiologists, neurologists, endocrinologists, gastroenterologists, pulmonologists, nephrologists, rheumatologists, infectious disease specialists, psychiatrists, addiction medicine physicians, dermatovenerologists, TB specialists, ophthalmologists, and ENTs. Dentists of all specializations, along with ultrasound, radiology, functional diagnostics, and laboratory physicians, are also Grade 13.

3. Real Income and the SSP

The base salary accounts for roughly 30–40% of actual take-home pay. The bulk of income comes from the federal SSP — Special Social Payment (специальная социальная выплата), which is tax-exempt and excluded from average-earnings calculations.

Working in a district hospital serving fewer than 50,000 residents ends up paying more than a position in a large Luhansk city clinic.

Note: SSP applies to primary care physicians — GPs, district pediatricians, and FAP (rural medical outpost) feldshers. Narrow specialists working in inpatient facilities do not receive SSP.

Estimated take-home for a district GP: base salary with allowances (~₽50,000) + SSP (₽50,000) = ~₽100,000/month (~$1,000).


4. Career Progression

Chief Physician salaries are fixed to hospital scale (Labor Payment Group):

  • Chief Physician, Republican Hospital (Group I): Grade 18 = ₽37,862 base + bonuses.
  • Chief Physician, District Hospital (Group II): Grade 17 = ₽35,388 base.
  • Head Nurse: Grades 10–12 (up to ₽25,000 base).

5. Housing

The LPR faces a structural mismatch: a shortage of good-quality housing stock alongside relatively low rental prices.

A subsidized mortgage program at 2% APR operates in the region. There is no centralized rental reimbursement scheme — the decision to cover housing costs rests with the Chief Physician of each individual facility and carries no guarantee.

6–8. Contract Terms, Internship Logistics, and Termination

No public documents guarantee travel or accommodation reimbursement for contract students (целевики) during internships. Students assigned to a facility in a district other than their place of study bear those costs personally.

The sponsoring organization (заказчик) for contract training is typically the LPR Ministry of Health. This creates a deferred-choice structure: the applicant signs with the Ministry, and the specific workplace — city clinic or rural district — is determined by a commission only after graduation. The student has no formal input at that stage.

The contract training agreement (целевой договор) mandates a mandatory service period (отработка) of 3 to 5 years. Personal circumstances and changes of plan do not constitute grounds for penalty-free termination. Refusing to fulfill the service obligation requires repaying the full tuition cost plus a state budget fine.


Pros and Cons

The LPR contract training program offers the highest lump-sum payout tier available in Russia — ₽2,000,000 (~$20,000) — backed by a transparent tariff-based salary structure that has been in effect since January 2025. The combination of a district GP’s base salary with allowances and the full ₽50,000 SSP reaches roughly ₽100,000 per month, which at current Luhansk rental prices (₽15,000–25,000 for a one-bedroom) makes real estate acquisition within 3–4 years a plausible scenario. Surgeons and general practitioners benefit from the elevated Grade 14 coefficient, and the administrative path to receiving Zemsky Doctor funds is simplified by the region’s staffing deficit: most facilities are at 30–40% capacity, removing the usual 60% threshold barrier.

The contract structure introduces constraints that are difficult to reverse. The LPR Ministry of Health, not the student, determines the placement city or district after graduation. Refusing a rural assignment or renegotiating terms carries no legal off-ramp without repaying full tuition and penalties. No regional settling-in bonus exists to supplement the federal payment. Housing compensation depends entirely on the individual Chief Physician’s decision and cannot be factored into planning with any certainty. Some remote districts face infrastructure limitations — water supply reliability, mobile connectivity — that are not reflected in the salary figures.

Signing a contract training agreement with the LPR Ministry of Health is a 3-to-5-year geographic commitment with real financial upside and real constraints. This investigation aims to give you the numbers to weigh both.
