Contract Training in Krasnodar Krai: ₽1.5M Mortgage Subsidy, Zemsky Doctor, and What You’ll Actually Earn

As a 2025 applicant seriously considering a medical university under a contract quota (целевая квота), I understood that this decision would shape the next 8–10 years of my life. Faced with a shortage of complete and honest information, I decided to go directly to the source. I formulated eight key questions about financial, housing, and contractual conditions and sent an official letter to the Ministry of Health of the Krasnodar Krai.
I received a detailed official response. The Ministry answered most questions thoroughly — particularly those about social support measures and the new admissions procedure. Questions about specific financial guarantees, however, were left without direct answers.
Below is a two-part analysis: first, what the Ministry told me and what their answers actually mean; second, what independent research through open sources revealed.
Part 1: What the Ministry of Health Told Me
Question 1: Zemsky Doctor — Does It Apply to Contract Graduates?
Ministry’s response: The Zemsky Doctor / Zemsky Feldsher program operates in Krasnodar Krai under RF Government Decree No. 1640. One-time compensatory payments are available to physicians who relocate to work in rural settlements, workers’ settlements, urban-type settlements, or cities with populations under 50,000. For physicians, payments range from ₽1,000,000 (~$10,000) to ₽1,500,000 (~$15,000); for mid-level medical staff, from ₽500,000 (~$5,000) to ₽750,000 (~$7,500). One eligibility condition stands out: applicants must have «no unfulfilled obligations under a contract training agreement (целевой договор)» — except when employed at facilities with less than 60% staff occupancy. Employment must occur in the same year as graduation.
What this means: The phrase about unfulfilled obligations is the crucial detail for contract students (целевик). In practice, this likely means Zemsky Doctor becomes accessible only after completing the mandatory service period (отработка) of 3–5 years, unless your assigned hospital falls below the 60% staffing threshold. Assignments to severely understaffed facilities open an earlier path to the payment — but come with correspondingly heavier workloads.
Question 2: Are There Settling-in Bonuses?
Ministry’s response: No direct answer on general settling-in bonuses (подъёмные) was provided. Instead, the Ministry described two mortgage-related social payments: ₽1,000,000 (~$10,000) for physicians across the Krai under Governor’s Decree No. 912, and ₽1,500,000 (~$15,000) specifically for Krasnodar city medical workers under Governor’s Decree No. 15 of January 27, 2025.
What this means: These are not cash payments to spend freely. Both go directly to a bank as a mortgage down payment. Each comes with an additional 5-year work commitment in the state healthcare system — separate from and on top of the standard contract training obligation. Combined, these timelines can reach 8–10 years of regional attachment.
Question 3: What Is the Base Salary?
Ministry’s response: No specific salary figures were provided.
What this means: The Ministry did not disclose a base salary grid. Finding one requires searching regional regulatory documents independently — which I did (see Part 2).
Question 4: What Will a Young Doctor Actually Earn?
Ministry’s response: No direct answer.
What this means: The Ministry neither confirmed nor denied any specific income level for recent graduates. Market analysis through job listings is the only reliable method, and the gap between official statistics and real starting pay is substantial (see Part 2).
Question 5: Which Housing Programs Apply — Including in Krasnodar City?
Ministry’s response: This was the most detailed section. Nine regional support measures were listed, covering the entire Krai including Krasnodar: service housing under Regional Law No. 1450-KZ; rent compensation under Decree No. 299 (₽5,000–₽20,000/month depending on location); utility bill compensation for rural staff under Decree No. 65; land plots for individual construction under Law No. 3232-KZ (up to 6 years, free use); a general mortgage down payment program under Decree No. 712 (for employees with 2+ years in public sector and 5+ years residency); savings account bonus of 30% up to ₽180,000 under Decree No. 824; the ₽1,000,000 mortgage subsidy under Decree No. 912; the ₽1,500,000 Krasnodar-city subsidy under Decree No. 15; and priority kindergarten placement for children of primary care and emergency medicine staff.
What this means: The housing package on paper is extensive. Actual availability varies sharply by program — service housing is limited, the land plot program is restricted to specific municipalities, and the flagship mortgage subsidies carry long work commitments.
Question 6: Is There Support During Student Internships?
Ministry’s response: Practice takes place at the future employer’s facility under a mentor. No mention of travel or accommodation costs.
What this means: Financial support during internships is not guaranteed at the regional level. Each hospital decides independently whether to provide dormitory space or any other assistance.
Question 7: When and How Is the Future Workplace Chosen?
Ministry’s response: Under RF Government Decree No. 555, the sponsoring organization (заказчик) — in this case the regional Ministry — publishes specific offers on the «Work in Russia» portal (trudvsem.ru) no later than June 10 each year. Each offer names the future employer. Applicants choose a specific offer when submitting their application.
What this means: The system has changed substantially. Before 2024, a contract student often signed an agreement without knowing which specific hospital they would end up in after graduation. Under the new procedure, the workplace is known before signing — which makes the decision far more concrete.
Question 8: Can the Contract Be Modified or Terminated?
Ministry’s response: A general reference to Decree No. 555, with no elaboration.
What this means: Federal law does provide mechanisms — but they require initiative from the graduate to invoke them (see Part 2).
Part 2: What I Found Through Independent Research
Zemsky Doctor: Amounts, Eligibility, and the 60% Rule
The Ministry confirmed payment amounts of ₽1,000,000 to ₽1,500,000 for physicians. Krasnodar Krai is not classified as Far North or a DFO territory, so the higher ₽2,000,000 rate does not apply here. The ₽1,500,000 figure likely corresponds to certain remote or hard-to-reach districts within the Krai; which specific municipalities qualify for this higher rate is not listed in a single accessible public document — confirmed vacancies in the program register must be checked on the «Work in Russia» portal and on individual CRH (Central District Hospital) websites.
For the Apsheron CRH, for example, the program register includes vacancies for general practitioners, oncologists, and urologists. The program has been extended through 2030 by presidential order.
The 60% staffing threshold deserves attention. Facilities below that level are exempt from the «no unfulfilled obligations» requirement — meaning a contract graduate assigned to such a facility could theoretically access Zemsky Doctor immediately. The tradeoff is a clinical environment operating at severe understaffing, where workload often reaches far beyond a standard schedule.
Settling-in Bonuses, Mortgage Subsidies, and SSP
There is no universal settling-in bonus program in Krasnodar Krai for all graduating physicians. The two mortgage subsidies described by the Ministry — ₽1,000,000 across the Krai and ₽1,500,000 for Krasnodar city — are the primary lump-sum support instruments, and both function as down payment transfers directly to a mortgage lender.
These are not interchangeable with cash. Receiving either subsidy adds a 5-year mandatory work commitment. A contract student completing a 5-year mandatory service period who also takes the ₽1,500,000 subsidy faces a total regional obligation of up to 10 years.
Separate from these programs, the Special Social Payment (SSP) operates as a monthly federal supplement paid through the Social Fund of Russia. SSP is tax-free and excluded from average-earnings calculations. For physicians working in settlements under 50,000 residents, SSP adds ₽50,000/month; in towns of 50,000–100,000 residents, it adds ₽29,000/month. SSP applies to primary care physicians — general practitioners, pediatricians, and rural outpost feldshers — not to narrow inpatient specialists. The Ministry’s letter did not mention SSP at all.
By the numbers for a rural GP: if the base «take-home» salary is ₽55,000, adding ₽50,000 in SSP brings monthly income to ₽105,000 (~$1,050). That already exceeds the regional average wage.
Base Salary and Real Income
No unified salary grid for Krasnodar Krai physicians is available in a single public document. Governor’s Decree No. 1152 of December 25, 2023, amending the regional remuneration system, establishes one concrete figure for young specialists: a 30% salary bonus during the first three years of employment, followed by 15% for each subsequent two-year period. This is a meaningful supplement that compounds over time.
For actual salary ranges, job listings on hh.ru provide the most realistic picture.

Official Rosstat figures for the Krai cite average nominal wages of approximately ₽78,000–₽79,000 by mid-2025. A newly graduated physician in a Krasnodar outpatient clinic (поликлиника) earns 20–35% below that average before SSP. With SSP in a sub-50,000 settlement, the picture reverses: total income can reach or exceed ₽105,000, above the regional mean.
The Rosstat figure of ₽96,000–₽115,000 sometimes cited for «healthcare workers» reflects experienced physicians, department heads, and combined rates from part-time employment arrangements — not the starting package for a new graduate.
Growth Trajectory After the Mandatory Service Period
Five to seven years of public-sector practice open substantially different options. In Krasnodar, general practitioners with 5+ years of experience command ₽150,000–₽300,000 in private clinics. In Sochi, experienced physicians in state facilities earn up to ₽115,000; private-sector offers for specialists reach ₽150,000–₽500,000. The mandatory service period, seen from this angle, is also a period of clinical experience accumulation and professional network-building.
Housing Programs — What They Actually Cover
Service housing under Regional Law No. 1450-KZ exists but is scarce. Regional news sources report 76 apartments allocated to medical staff in all of 2024 and 17 in the first half of 2025 — across a region where hundreds of physicians enter the workforce each year. This program reaches the most in-demand specialists in the highest-shortage areas; counting on it as a baseline is unrealistic.
The land plot program under Law No. 3232-KZ applies only to specific municipalities and specialties. No consolidated public list exists; individual district administrations hold this information.
The rent compensation program under Decree No. 299 is geographically tiered. The practical question is how much of actual market rent these amounts cover.

In both Sochi and Krasnodar, the compensation covers roughly 40% of actual rent. A physician in Sochi, despite receiving the highest compensation in the region, still pays approximately ₽30,000 out of pocket each month on rent alone. In Krasnodar, the out-of-pocket gap is around ₽15,000.
The mortgage subsidies are the flagship programs by scale and by demonstrated uptake: more than 100 Krasnodar medical workers have received the ₽1,500,000 payment, and 429 regional workers have received the ₽1,000,000 payment since 2022, according to regional press releases.
Internship Support During Training
No regional program for reimbursing travel or accommodation costs during student internships was identified in public sources. In Krasnodar Krai, this appears not to be standard practice — unlike some other Russian regions that have introduced such mechanisms. The Ministry’s letter confirms only that all practical training takes place at the future employer’s facility. Whether individual hospitals provide dormitory access is negotiated case by case when signing the contract training agreement.
Choosing a Workplace
The new federal procedure under Decree No. 555 took effect on May 1, 2024. All offers for contract training must be published on the «Work in Russia» portal (trudvsem.ru) by June 10 of the admission year. Each offer names the specific healthcare institution as the future employer — not just «a facility in Krasnodar Krai.»
Applicants need to monitor trudvsem.ru from late May through early June. The practice of signing an agreement with the Ministry without knowing the specific assignment hospital is no longer standard under this system. For applicants going through the regional Ministry’s contract quota — as opposed to signing directly with a specific CRH — it is still worth confirming at the time of signing exactly which facility will be listed as employer.
Modifying or Terminating the Contract
Changing the place of work within the region is legally possible but requires a tripartite agreement: the sponsoring organization (the Ministry), the current employer, and the prospective new employer must all consent, after which a supplementary agreement is appended to the original contract.
Penalty-free termination is governed by Section V of Decree No. 555. Valid grounds include assignment of Group I or Group II disability to the graduate, their spouse, a parent, or a child; caregiving obligations toward a close relative with Group I disability; a spouse’s relocation due to military contract service; and other circumstances listed in the Decree. These federal protections exist regardless of what the regional contract says; they apply uniformly to all contract students in Russia.
Pros and Cons
Contract training in Krasnodar Krai is, on balance, a well-supported program — but the financial picture is more complicated than the headline numbers suggest.
The case for signing: the mortgage subsidy system is the strongest feature. Grants of ₽1,000,000 and ₽1,500,000 as down payments solve the hardest problem young physicians face — housing — faster than saving on a doctor’s starting salary would allow. SSP supplements of up to ₽50,000/month in rural areas bring real income above regional averages for primary care physicians from the first year. The 30% young-specialist bonus in the first three years adds further to early-career compensation. The mandatory Zemsky Doctor program runs through 2030, and the new trudvsem.ru platform makes the workplace selection process genuinely transparent for the first time.
The case against: starting salaries in Krasnodar city outpatient clinics run 20–35% below the regional mean, creating real financial pressure during the first years. Accessing Zemsky Doctor on top of the contract training obligation means a realistic total timeline of 8–10 years of regional commitment, roughly matching or exceeding the length of a standard mortgage. Rent compensation covers only 40% of actual market prices in Krasnodar and Sochi, and service housing allocations are far too limited to serve as a reliable fallback. SSP is available only to primary care physicians, not to all specialties.
The core tradeoff is clear: fast, partially subsidized access to homeownership and a structured early career, in exchange for a decade of commitment to the Krasnodar Krai state healthcare system.
