How a Contract Physician in the Republic of Altai Can Earn ₽155,000 and Get Full Rent Compensation

The Republic of Altai is a region where 2025 produced an unusual reversal: federal supplements made rural positions more lucrative than working in the regional capital. Authorities responded with support measures rare for Siberia, effectively closing the income gap between the city and the countryside.
This article is a detailed breakdown of conditions for future physicians, prepared as part of the «Navigator for Contract Students» project. The aim is to check whether the ambitious statements from regional officials hold up against the actual regulatory documents.
Note on the Official Response
At the time this article was prepared, no official response had been received from the Ministry of Health of the Republic of Altai. The analysis below is based entirely on open official sources: regional and federal regulations, Rosstat statistics, job portals, and the rental market.
1. Zemsky Doctor and Zemsky Feldsher Programs
The Republic of Altai has complex terrain, and the program operates on a differentiated scale across the region.
Two districts are equated to the Far North — Kosh-Agachsky and Ulagansky. Physicians working there receive ₽2,000,000 (~$20,000) as a lump-sum grant; feldshers receive ₽1,000,000 (~$10,000). All other rural territories of the republic follow the standard rates: ₽1,000,000 (~$10,000) for physicians and ₽500,000 (~$5,000) for feldshers.
One condition deserves attention before signing anything. Under regional rules, the Zemsky Doctor payment is available only when joining a medical organization whose staffing level is below 60%. A contract student (целевик) assigned to a district hospital with 65% staffing will be denied the grant outright. Verify the staffing rate of any prospective employer before the contract training agreement (целевой договор) is signed.
2. Settling-in Bonuses and Regional Supplements
Until 2025, physicians in Gorno-Altaysk were at a financial disadvantage relative to rural colleagues who received federal supplements. No regional settling-in bonus (подъёмные) was found in open sources for that period.
Starting January 1, 2025, the situation changed. The regional government introduced Regional Special Social Payments for medical staff in Gorno-Altaysk, payments that exist outside the federal list. Physicians receive a monthly top-up of ₽50,000; nursing and mid-level staff receive ₽30,000. This functions as a «capital city allowance,» making the republic’s only city financially competitive with rural postings for the first time.
3. Base Salary and Salary Structure
The compensation system is in transition. In 2024, Ministry of Health Order No. P-02-01/0056 revised salary structures in line with the «50% rule,» under which the guaranteed base salary must constitute at least half of total compensation.
According to hh.ru data from autumn 2025, the average offered physician salary in the Republic of Altai is approximately ₽100,000/month, second in the Siberian Federal District, behind the Tuva Republic.

4. Real Income: City vs. Countryside
Two scenarios capture the financial reality for a newly graduated physician.
Working in Gorno-Altaysk. Net take-home salary runs approximately ₽65,000 after tax. Add the new regional supplement (₽50,000) and rent compensation (₽40,000, treated here as indirect income). The total economic value of this package reaches ₽155,000/month (~$1,550).
Working in a rural settlement. The SSP — Special Social Payment (ССВ) — is mandatory here. For settlements with under 50,000 residents, primary care physicians receive ₽50,000/month in SSP on top of their salary. Combined income reaches ₽135,000–150,000/month (~$1,350–1,500), though infrastructure and everyday comfort are lower than in the city.
The SSP is tax-free. It applies to GPs, pediatricians, general practitioners, and FAP feldshers. Narrow specialists in inpatient facilities do not receive it.
5. Housing Programs
A tourism boom has pushed Gorno-Altaysk rental prices to levels comparable with much larger Russian cities. A one-bedroom apartment starts at ₽30,000/month; a three-bedroom runs from ₽48,000.
In November 2024, Decree No. 376 raised the rent compensation ceiling. Physicians signing their first employment contract receive up to ₽40,000/month. Those re-employed at a new facility receive ₽20,000.

Full coverage for a one- or two-bedroom apartment is unusual by Russian state program standards, where partial coverage of 17–45% is the norm.
6. Internship Costs: What the Documents Don’t Mention
The Republic of Altai has no medical university of its own. Students enroll in neighboring regions: Altai Krai (Barnaul), Tomsk, or Novosibirsk.
No regional regulations covering travel or accommodation reimbursement during mandatory internships were found. Every cost falls on the student’s family. The region’s popularity with tourists makes short-term rentals expensive.

Over six years of study with mandatory internships, total out-of-pocket costs can exceed ₽200,000 (~$2,000).
7. Choosing a Workplace
Since May 1, 2024, the placement procedure has been governed by a new federal framework operating through the «Work in Russia» portal (trudvsem.ru).
Pay attention to who the sponsoring organization (заказчик) is. When the Ministry of Health of the Republic of Altai acts as the sponsoring organization, the specific medical facility may not be named at the time of signing. With a ₽50,000 monthly city bonus now in place, competition for postings in Gorno-Altaysk will be higher than before. An applicant may be directed to remote mountain districts such as Ulagan or Kosh-Agach, where Zemsky Doctor payments are elevated to ₽2,000,000 but daily living conditions are considerably harsher.
8. Contract Terms: Transfers and Termination
No regional regulations were found that simplify transferring between facilities during the mandatory service period (отработка). Changing hospitals requires mutual agreement from both parties — difficult to obtain when the region is short-staffed.
Federal law, Government Decree No. 555, permits penalty-free contract termination in specific circumstances: establishment of Group I or II disability, the need to care for a Group I disabled family member, or relocation necessitated by a military spouse’s transfer. Outside these grounds, early exit triggers full repayment of study costs and, where applicable, Zemsky Doctor funds already received.
Pros and Cons
For
The Republic of Altai offers some of the highest financial support in Siberia. Rent compensation of up to ₽40,000/month fully covers market rates for a one- or two-bedroom apartment, something few state programs can claim. The January 2025 regional supplement (₽50,000/month) eliminated the income gap between city and village, giving contract students a genuine choice between two financially viable options. The natural environment and air quality are a genuine advantage for physicians planning to settle long-term.
Against
The absence of a local medical university means four to six mandatory internship trips, with all travel and accommodation costs borne by the student or family. Tourism-driven rental prices in Gorno-Altaysk erode the advantage of high incomes for physicians who don’t qualify for the housing supplement. The Zemsky Doctor grant is off the table for anyone placed in a hospital with staffing above 60%, a condition worth verifying with specific facilities, not just the ministry. The region has no rail connection, which limits logistics for both daily life and emergencies.
