Physicians in Ingushetia: Up to ₽89,000 Take-Home, a Mobile Zemsky Million, and Bonuses for Red Diploma Graduates

Analysis of the Official Response
At the time of publication, the editorial team has not received an official response from the Ministry of Health of the Republic of Ingushetia (RI). The analysis below draws on regional regulatory acts — Government Decrees of RI No. 91, No. 81, and No. 75 — and labor market data from open sources.
Independent Research
1. Support Programs — Zemsky Doctor
The Republic of Ingushetia is not classified as a Far North or equivalent territory under Government Decree No. 1946, nor does it fall within the Far Eastern Federal District or the Arctic zone. As a result, the standard federal Zemsky Doctor payment applies: ₽1,000,000 (~$10,000) for physicians and ₽500,000 (~$5,000) for feldshers working in rural settlements (Government Decree of RI No. 81).
The payment is available to contract students (целевики) immediately upon employment, subject to one condition: the receiving facility’s staffing level must be below 60%.
A 2024 amendment to Decree No. 81 introduced a notable clause. A physician may now change their place of employment once — transferring to another rural hospital within the republic — without repaying the lump sum. The option requires mutual agreement with both facilities but carries no financial penalty. In practice, this means a new doctor who finds the working environment at the initial assignment difficult can seek a transfer without losing the million rubles.
2. Settling-in Bonuses (подъёмные)
No regional regulatory acts were found establishing one-time payments at the point of employment beyond the Zemsky funds. Rural financial support is instead provided through the federal Special Social Payment (SSP — специальная социальная выплата).
In settlements with under 50,000 residents, physicians receive ₽50,000 per month, and mid-level medical staff receive ₽30,000 per month (Government Decree RF No. 2568). Across twelve months, the SSP alone totals ₽600,000. The payment is tax-free and excluded from average-earnings calculations.
SSP applies to primary care physicians — general practitioners, district internists, and pediatricians working in outpatient clinics and FAPs (rural medical outposts). Narrow specialists employed primarily in inpatient settings do not receive it.
3. Base Salary
The salary structure is governed by Decree of the Government of RI dated June 1, 2016, No. 91 (amended September 27, 2022). Appendix No. 2 sets out rates by qualification level:
- Intern physician, Level 1: ₽19,450
- Outpatient specialist, Level 2: ₽22,600
- District general practitioner or pediatrician, Level 3: ₽22,700
- Inpatient physician or surgeon, Levels 3–4: ₽22,700–₽22,810
These figures represent the base before any allowances, taxes, or supplements.
4. Real Income — What You Will Actually Earn
Vacancy data from GorodRabot.ru (2025) shows advertised salaries for district general practitioners reaching up to ₽80,000. That figure comes from a layered system of allowances defined in Decree No. 91.
Compensatory payments (guaranteed): A hazardous conditions supplement of at least 4% of base salary applies following a workplace conditions assessment. An additional 25% of base salary is paid as a risk supplement for work involving infectious disease exposure, per Appendix 9 to Decree No. 91.
Incentive payments: Qualification categories add 5% (second category), 15% (first category), or 20% (highest category) to the base. A candidate of sciences degree adds 30%. Facility directors have the authority — not an obligation — to pay an extra 10% to young specialists who graduated with honors (red diploma).

Career trajectory: Income growth through seniority is constrained. The longevity bonus for physicians caps at 20% after five years of service. For comparison, ambulance drivers in the same system have a progressive seniority scale reaching 80% of base salary. The practical path to higher earnings for a physician is obtaining a qualification category and working at 1.5 rates, which can raise monthly income to the ₽120,000–₽150,000 range.
5. Housing
Open sources contain no evidence of a mass-scale service housing program with privatization rights in the republic. Support is limited to utility compensation under Decree of the Government of RI No. 75. For rural doctors, the fixed compensation for heating and electricity costs is ₽1,200 per month.

6. Practical Training (Internships)
Regional regulations provide no reimbursement for travel or accommodation during mandatory internship placements. The costs fall to the family.
One form of support exists for high-achieving students: the Scholarship of the Head of RI (Edict No. 170), worth ₽4,000 per month. The quota covers only 5 students among all those studying outside the republic — making it more a token recognition than practical financial relief.

Four mandatory internship placements across the course of study translate to approximately ₽112,000 in out-of-pocket expenses at these rates.
7. Choosing a Place of Work
Placement follows the federal procedure via the «Work in Russia» portal (trudvsem.ru), in force since May 1, 2024. The sponsoring organization (заказчик) in most Ingushetia contracts is the regional Ministry of Health rather than a specific facility, which means the actual workplace is not fixed at the time of signing the contract training agreement (целевой договор).
Beyond the online procedure, the regional ministry maintains a practice of in-person document submission at its Nazran office. Reception hours are frequently limited to 10:00–13:00. Applicants should monitor the «Personnel Policy» section of the ministry’s website for current deadlines and any changes to the process.
8. Contract Terms
Workplace transfer: No specific regional regulation governs mid-contract transfers. The mobility clause in the Zemsky Doctor program (Decree No. 81, 2024 edition) provides a de facto mechanism — a physician can move to a different rural hospital by mutual agreement without repaying the lump sum — but this applies specifically to Zemsky recipients and does not constitute a general transfer procedure.
Contract termination: Governed by Federal Government Decree No. 555. Penalty-free termination is permitted in three circumstances: the student or graduate is assigned a Category I or II disability; care for a close relative with a Category I disability becomes necessary; or a military spouse is transferred to a new duty station.
Pros and Cons
Ingushetia offers a functional entry point into state-sector medical employment, with a compensation structure that outperforms first impressions. The federal SSP closes much of the gap between the low base salary and a livable income, and the Zemsky Doctor mobility clause is a concrete practical advantage unavailable in most other regions.
On the positive side: the base salary of ₽22,700 is above the floor for several other Russian regions, and the combination of SSP plus allowances brings take-home pay to roughly ₽89,000 for a newly qualified district GP — without requiring any additional specialization or seniority. The 10% bonus for honors graduates is formalized in regional law, not left to managerial discretion. The ability to transfer between rural hospitals under the Zemsky program without losing ₽1,000,000 gives contract students a degree of flexibility that most comparable programs do not offer.
The constraints are also specific. Utility compensation at ₽1,200 covers around 5–6% of market rent in Nazran or Magas, making housing costs essentially a personal expense. The seniority ceiling at 20% after five years limits long-term income growth significantly — ambulance drivers in the same system accumulate a 80% seniority supplement under a progressive scale, which the physician salary structure does not replicate. Internship travel and accommodation — roughly ₽28,000 per placement — fall entirely on the family budget, requiring financial reserves that not every household has.
Signing a contract training agreement for Ingushetia makes financial sense for candidates who qualify for SSP-eligible primary care positions and who value contract portability. Candidates expecting formal housing support or strong seniority-based income growth will find the republic’s offering lean on both counts.
