Contract Training in North Ossetia: ₽1.5M Settling-in Bonus the Ministry Forgot to Mention

Note: As of 2025, 1 USD ≈ 100 RUB. All figures are in Russian rubles (₽) unless otherwise stated.

The Ministry of Health of the Republic of North Ossetia-Alania produced one of the more useful official responses we received — unusual because it came with an attached salary table, the document that normally takes the longest to track down. Below, the Ministry’s position on each question is set against what the regulatory record actually shows.


Part 1: What the Ministry of Health Told Me

Question 1: Zemsky Doctor

Ministry’s response: The program is confirmed for 2026–2028. However, the subsidy is only available to doctors who have no outstanding financial obligations under an existing contract training agreement (целевой договор).

What this means: A contract student (целевик) who graduates and goes straight to a rural posting cannot receive Zemsky Doctor money until the mandatory service period (отработка) under the contract training agreement is fully discharged. In practice, if the contract runs six years of study plus three years of service, Zemsky eligibility begins only after year nine.


Question 2: Settling-in Bonus

Ministry’s response: Regional lump-sum payments upon employment are not provided.

What this means: At the time the Ministry signed this letter, that was formally correct. A regional decree published two months later changed the picture entirely — see Part 2.


Question 3: Base Salary

Ministry’s response: No single figure was given. The Ministry referenced Order No. 1178 o/d (dated December 29, 2017), which sets recommended base salary levels by professional qualification group (PQG). The starting rate for a Level 2 specialist doctor under this order is ₽18,779.

What this means: That figure is the guaranteed floor. Everything above it — incentive payments (стимулирующие выплаты), compensatory payments (компенсационные выплаты), category bonuses, seniority allowances — is discretionary and set by each facility independently within its payroll budget.


Question 4: Real Income

Ministry’s response: No guaranteed income figure was provided. The Ministry gave an estimated «net» range of ₽30,000 to ₽50,000 for young doctors in district and city hospitals.

What this means: That range is plausible for Vladikavkaz. For a doctor in a small town or village, the federal Special Social Payment (SSP — Special Social Payment) shifts the calculation substantially. The Ministry did not mention SSP at all.


Question 5: Housing

Ministry’s response: State program Decree No. 277 (July 18, 2023) offers a targeted subsidy toward a mortgage down payment and partial interest compensation. Service housing (служебное жильё) may also be available through the facility directly, subject to stock availability.

What this means: Both mechanisms exist on paper. Quota limits mean neither is guaranteed — the 2025 cohort for the housing subsidy program covers roughly 40 specialists across the entire republic.


Question 6: Practical Training

Ministry’s response: No compensation for travel or accommodation during clinical placements is provided.

What this means: All logistics costs fall to the student. A single placement cycle from Moscow to Vladikavkaz costs an estimated ₽35,000–₽50,000 in airfare or rail tickets plus one month’s rent.


Question 7: Workplace Selection

Ministry’s response: The specific facility is determined after graduation based on current staffing needs. Choosing an employer before signing a contract training agreement is not permitted.

What this means: The Ministry’s position contradicts federal law in force since May 1, 2024. Government Decree No. 434 requires a specific vacancy to be selected on the «Work in Russia» portal (trudvsem.ru) before the contract is signed. The Ministry has not updated its procedure to reflect this.


Question 8: Contract Terms and Termination

Ministry’s response: No internal procedure exists for changing a placement site within the region. Early termination is governed exclusively by Government Decree No. 555 (April 27, 2024).

What this means: The Ministry is effectively saying it decides placements unilaterally, with no formal channel for the student to request a transfer. Federal Decree No. 555 does protect students in specific situations — see Part 2.


Part 2: What I Found Through Independent Research

Zemsky Doctor — Amounts and Quotas

The standard Zemsky Doctor payment in North Ossetia is ₽1,000,000 (~$10,000). For villages classified as remote or hard-to-reach under Decree No. 263 (July 25, 2025), the rate rises to ₽1,500,000 (~$15,000). The Mozdok district settlements named in that decree include the stanitsa Pavlodolskaya and the village Predgornoye.

Two caveats apply. First, only specific settlements within Mozdok district qualify for the higher rate — the rest of the republic falls under the standard ₽1,000,000 tier. Second, the program for 2025 funds only 25 positions. Moving to a qualifying village does not guarantee entry into the registry; demand consistently exceeds available slots.

Settling-in Bonus (подъёмные) — The Decree That Arrived After the Letter

Government Decree No. 391 of the Republic of North Ossetia-Alania, dated October 15, 2025, introduced regional lump-sum settling-in bonuses (подъёмные) that were not mentioned in the Ministry’s response because the decree postdates the letter.

Accepting this bonus carries a double obligation: a five-year work requirement under the bonus agreement runs in addition to, not instead of, the original mandatory service period under the contract training agreement. Clause 4(1) of the Decree creates one opening: a contract student with an outstanding contract training agreement can still receive the bonus if the target facility’s staffing level is below 60%.

Base Salary — Why the Floor Matters

Order No. 1178 o/d establishes recommended base salaries, not guaranteed ones. The appendix provided with the Ministry’s response lists the following for the «Physicians and Pharmacists» qualification group:

  • Level 1 (resident/trainee): ₽17,051
  • Level 2 (specialist doctor): ₽18,779
  • Level 3 (inpatient, district GP/pediatrician): ₽19,795
  • Level 4 (surgical specialists, anesthesiologist-intensivist, pathologist): ₽21,321

A base salary of ₽18,779 is roughly half the regional average wage, which makes the doctor financially dependent on whatever incentive payments the facility chooses to award each month.

Real Income — The SSP Gap Between City and District

The federal SSP creates a pronounced income difference between Vladikavkaz and rural postings. SSP is tax-free and excluded from average-earnings calculations.

Career trajectory varies sharply by specialty and location. A surgeon at the Mozdok Central District Hospital (ЦРБ — CRH) can realistically reach ₽100,000 or above once seniority and category bonuses accumulate. A district GP in the Vladikavkaz outpatient network rarely clears ₽50,000–₽60,000, regardless of years on the job.

Housing — Costs Relative to Salary

Vladikavkaz rental prices are high relative to what a starting doctor actually earns in the city.

The housing subsidy program under Decree No. 277 offers up to ₽1,000,000 (~$10,000) toward a mortgage down payment. Given that the 2025 cohort covers approximately 40 specialists across the republic, competition for slots is real.

Practical Training — Hidden Cost Calculation

With no compensation in place, four mandatory clinical placements over the course of a six-year spetsialitet degree add up. One placement from Moscow to Vladikavkaz costs roughly ₽15,000–₽25,000 in transport plus ₽20,000–₽25,000 for a month’s accommodation — a total of ₽35,000–₽50,000 per trip at minimum.

Workplace Selection — The Federal Rule the Ministry Ignores

Since May 1, 2024, Government Decree No. 434 requires contract students to choose a specific vacancy listed on the «Work in Russia» portal (trudvsem.ru) before signing. The sponsoring organization (заказчик) in North Ossetia is the regional Ministry of Health, not a named hospital — which means that at the time of signing, the student has no confirmed placement site. The Ministry’s response treats this as standard procedure. Under current federal rules, it is not.

Contract Termination — What Federal Law Actually Permits

Government Decree No. 555 allows penalty-free termination of a contract training agreement in several defined situations. These include establishing a Group I or II disability, the need to provide full-time care for a first-degree relative with a Group I disability, and compulsory relocation following a military spouse’s reassignment. The Ministry confirmed that no regional transfer mechanism exists within the republic, meaning that if your assigned posting is inconvenient, the only formal exit is one of these federal grounds.


Pros and Cons

North Ossetia-Alania presents a case where the official response and the actual regulatory environment diverge in ways that directly affect how much a contract student can earn and what obligations they take on.

The financial case for a rural or small-town posting is real. Decree No. 391 introduces settling-in bonuses up to ₽1,500,000 (~$15,000) for remote settlements — money the Ministry’s letter did not mention. The federal SSP of ₽50,000 per month in towns under 50,000 residents pushes combined income to ₽70,000–₽80,000, well above what Vladikavkaz offers. Understaffed facilities — those below 60% staffing — allow contract students to access these bonuses even while an existing contract training agreement is outstanding, which is a material flexibility.

The disadvantages concentrate on duration and legal clarity. Accepting the regional bonus locks the doctor in for an additional five years on top of the original mandatory service period, a combined timeline that can run eight to ten years. The Ministry continues to assign graduates by administrative discretion rather than following the federal «Work in Russia» portal procedure, meaning the actual facility remains unknown at the time of signing. And Vladikavkaz specifically is an economic dead end for starting physicians: low base salaries, no SSP at maximum rates, and rent consuming up to 85% of monthly income.

The decision to sign a contract in this republic is worth making with Decree No. 391 in hand, not the Ministry’s letter alone.
