Contract Doctor in Kamchatka: ₽2,000,000 Zemsky Payment, 2% Mortgage, and What the Numbers Hide

Question 1. The Zemsky Doctor Program
Under Government Decree No. 1946, the entire territory of the Kamchatka Territory is classified as Far North. This triggers the maximum federal payment rate for all qualifying settlements across the region — no district-by-district distinctions apply here.
Doctors who take up positions in rural settlements, work settlements, or towns with fewer than 50,000 residents receive a one-time lump sum of ₽2,000,000 (~$20,000). For feldshers, the Zemsky Feldsher equivalent is ₽1,000,000 (~$10,000). The contract runs for five years. Staffing levels at most regional facilities are below 60%, which means the 60% eligibility threshold does not block access to the program in practice.
There is a nuance that applicants frequently miss. The regional capital, Petropavlovsk-Kamchatsky, has a population of over 160,000 — well above the 50,000 threshold. Employment at the Regional Hospital, the Oncology Center, or the Perinatal Center does not qualify for Zemsky Doctor. Anyone targeting the capital’s flagship institutions receives no lump-sum payment.
The satellite city of Yelizovo, located 25–30 km from Petropavlovsk-Kamchatsky, is an exception. Its population falls under the 50,000 threshold, so employment at the Yelizovo CRH (Central District Hospital) qualifies for the full ₽2,000,000 payment.
One more thing about that ₽2,000,000: on the Kamchatka real estate market, it currently represents 30–40% of the price of a standard one-bedroom apartment. A solid down payment, but not a standalone housing solution.
Question 2. Settling-in Bonuses and Social Payments
No regional settling-in bonuses (подъёмные) — one-time payments made directly by the regional budget at the point of hire — were found in open sources. This does not appear to be a program the Kamchatka regional government runs separately from federal mechanisms.
The federal Special Social Payment (SSP) introduced by Government Decree No. 2568 is, however, a meaningful monthly supplement. The amount depends on where the doctor works. Doctors in small towns and rural settlements — Yelizovo, Vilyuchinsk, remote villages — receive ₽50,000/month net. Doctors in Petropavlovsk-Kamchatsky can claim ₽14,500/month, and only those working in primary care; narrow specialists at inpatient facilities in the capital do not receive SSP at all.
The monthly income gap between working in a small town and working in the capital, counting only SSP, is ₽35,500. That figure does not include the one-time Zemsky Doctor payment, which widens the gap further.
Question 3. Base Salary
A Decree of the Government of the Kamchatka Territory dated October 21, 2025 (No. 448-P) sets the salary scales for regional healthcare workers. The document confirms what is common across Russian regions: a low formal base with large supplementary coefficients layered on top.
The minimum base salaries for physicians (Occupational Qualification Group «Doctors and Pharmacists») are ₽14,505 for a trainee doctor (Level 3.1), ₽15,701 for a district GP or district pediatrician (Level 3.3), and ₽18,090 for surgical specialists and anesthesiologists (Level 3.4).
For a district-level therapist, the guaranteed portion of pay — base salary multiplied by the regional coefficient and northern bonus, before incentive payments — comes to approximately ₽40,800 (₽15,701 × 2.6). Everything above that, up to the ₽100,000+ figures quoted in vacancy listings, comes through incentive payments that are not contractually guaranteed.
Question 4. Real Income
Two multipliers shape what doctors actually earn in the Far North, and Kamchatka runs them at or near their legal maximums.
The regional multiplier is 1.8 across most of the territory (meaning a base salary of ₽100 becomes ₽180 in total), rising to 2.0 in the Aleutian District. The northern bonus (popularly called «Polyarki») reaches a maximum of 80% of the base salary, accrued progressively over years of service.
Doctors under 30 (in some cases under 35) who have already lived in a Far North region for at least five years receive the full 80% northern bonus from day one. For contract students arriving from central Russia («from the mainland»), the northern bonus typically accrues over time, though facilities commonly bridge the gap through the incentive payment structure of the effective contract to offer a competitive starting package immediately.

By nominal salary, Kamchatka ranks in the top six Russian regions. The purchasing power story is different. A liter of milk costs ₽120–180; vegetables in winter reach ₽800–1,000 per kilogram. ₽200,000 in Kamchatka buys roughly what ₽80,000–100,000 buys in central Russia.
Question 5. Housing
The rental market is strained. Demand from fly-in-fly-out workers, military personnel, and a chronic shortage of new residential construction have pushed prices well above what official compensation guidelines assume.

The most substantial housing tool available to a contract student (целевик) in Kamchatka is the Far Eastern Mortgage at 2% per annum. Monthly payments under this program are often lower than market rent. For doctors, it is available without age restrictions after five years of work in the Far Eastern Federal District (DFO). Young families — both partners under 36 — can access it immediately.
The program works differently depending on location. In small towns and rural settlements (including Yelizovo), the 2% mortgage applies to secondary-market housing. In Petropavlovsk-Kamchatsky, it applies only to new construction, which is scarce and expensive.
Question 6. Internship Costs
Standard contracts and open sources contain no guarantees of travel reimbursement for internship placements. A student enrolled in Moscow or Khabarovsk is contractually required to complete practical rotations with the sponsoring organization (заказчик) in Kamchatka. The geography makes this expensive.

Over six years of study, a family’s logistics costs can exceed ₽500,000 (~$5,000). Travel reimbursement must be negotiated as an explicit clause before the contract is signed — not assumed after the fact.
Question 7. Choosing a Workplace
Since May 1, 2024, state assignment (распределение) for contract students takes place through the «Work in Russia» portal (trudvsem.ru). The practical implications depend on who the sponsoring organization is.
If the sponsoring organization is a specific facility — say, the Ust-Kamchatka District Hospital — the workplace is fixed from the start. If the sponsoring organization is the Kamchatka Ministry of Health as a body, the actual posting is determined closer to graduation. That creates a real possibility of being assigned to the Penzhinsky or Olyutorsky districts, which are reachable only by helicopter or An-26 aircraft.
Intra-regional logistics in Kamchatka are weather-dependent to an extent that is easy to underestimate. «Non-flying» weather conditions in the remote north can ground planes for weeks. A student completing a practical rotation in such a district faces a genuine academic risk: if flights are delayed, returning to university by the semester start date may be physically impossible, which can create problems with the academic office.
Question 8. Contract Terms
Workplace transfers within the region are technically possible with mutual consent, since the regional Ministry of Health frequently acts as the sponsoring organization across multiple facilities. In practice, moving from a critically understaffed remote posting to Petropavlovsk-Kamchatsky is rarely approved.
Contract termination is governed by Government Decree No. 555 of April 27, 2024. Penalty-free exit is available on the following grounds: disability (Group I or II), the need to provide full-time care for a disabled relative, or the relocation of a military spouse. A straight voluntary exit requires repayment of the full cost of tuition. In Kamchatka, this is compounded by the obligation to repay any social support the region provided during training — flights, dormitory accommodation, stipends. Those sums are indexed to inflation and must be returned in full.
Pros and Cons
Contract training in Kamchatka is the highest-paying option the Russian federal program structure can produce, but that statement needs to be read carefully alongside the cost of living.
The case for Kamchatka rests on three concrete advantages. The Zemsky Doctor payment reaches ₽2,000,000 (~$20,000) because the entire region is classified as Far North — there are no partial-qualification caveats. SSP in small towns and rural areas adds ₽50,000/month net on top of base salary and multipliers, pushing total starting income in a district facility to ₽150,000–170,000/month or more. The Yelizovo option (the «golden route») lets a doctor live 20–30 minutes from the regional capital — with urban transport and services — while collecting all rural financial benefits: the ₽2M lump sum, ₽50,000/month SSP, and access to the secondary-market Far Eastern Mortgage at 2%.
The case against is equally concrete. High food and consumer prices reduce the real purchasing power of northern salaries to roughly half what the nominal figure suggests by central Russian standards. Internship logistics are a multi-year financial burden that falls entirely on the student and family if travel reimbursement is not negotiated into the contract. Working in Petropavlovsk-Kamchatsky — where the most complex clinical cases and best-equipped facilities are concentrated — means forfeiting both the Zemsky Doctor payment and the higher SSP tier, while paying the same high prices. For doctors posted to the remote northern districts, weather-dependent access to the rest of the region is a practical constraint that extends beyond vacation planning.
The decision to sign a Kamchatka contract is a five-year logistical and financial commitment made largely from a distance. This analysis exists to make that commitment legible before the signature goes on the page.
