Magadan Region Contract Training: ₽2M Zemsky Doctor, Immediate Northern Bonuses, and Annual Flight Reimbursement
This article is part of the Navigator for Contract Students project, a systematic investigation of contract training agreements across Russia’s 85 regions. For the Magadan Region, we apply the same eight-question framework used in every regional study: Zemsky Doctor eligibility, financial incentives, real salaries, housing programs, internship costs, workplace selection, and contract modification rules.
The official response carries the date «August 22, 2015,» but references regulatory acts from 2024. The date is a clerical error; the content is current.

Part 1: What the Ministry of Health Told Me
Question 1: Zemsky Doctor
Ministry’s response: The Ministry confirmed that contract students (целевики) are eligible for the Zemsky Doctor program. Physicians moving to settlements with fewer than 50,000 residents receive ₽2,000,000 (~$20,000); feldshers, midwives, and nurses at rural medical outposts (FAP) and feldsher-obstetric stations in the same categories of settlements receive ₽1,000,000 (~$10,000). Payment is made within 60 working days of signing the agreement. A mandatory service period (отработка) of five years applies.
What this means: The entire Magadan Region is classified as Far North under Government Decree No. 1946, so the maximum federal payment tier applies. However, the 50,000-resident cap is decisive: Magadan city itself, with approximately 90,000 residents, does not qualify. Contract students assigned to Magadan city hospitals receive no Zemsky Doctor payment.
Question 2: Settling-in Bonus
Ministry’s response: Under Regional Decree No. 159-pp of March 3, 2022, the region pays a one-time settling-in bonus (подъёмные) upon employment. Primary care and emergency physicians receive ₽300,000 (~$3,000); hospital physicians receive ₽200,000 (~$2,000). The mandatory service period for this payment is three years.
What this means: The bonus structure contains a counterintuitive logic. Physicians in Magadan city (where costs are highest) receive the larger amount (₽300,000), while those assigned to remote settlements receive less (₽200,000). The reason: physicians in remote areas qualify for the ₽2,000,000 Zemsky Doctor payment, so the region partially compensates Magadan-based physicians who are excluded from it. The compensation is partial and incomplete.
Question 3: Base Salary
Ministry’s response: The letter directed us to the institution’s internal remuneration regulations rather than citing specific figures.
What this means: We cross-referenced Regional Decree No. 535-pp of June 27, 2014 (as amended October 14, 2024), which sets binding salary floors. The minimum base salary for a first-qualification-level physician is ₽35,817; for second-level physicians, ₽38,958. These are floors — actual take-home pay is substantially higher once bonuses are applied.
Question 4: Real Income and SSP
Ministry’s response: The Ministry confirmed that young specialists receive the full northern bonus from their first working day, citing paragraph 8.7 of Decree No. 535-pp. If an employee has zero accumulated service in the Far North (and therefore a 0% personal northern bonus), the payroll office is legally required to add a compensatory supplement of 80% of base salary in Magadan city, or 100% in the Severo-Evensky District. The Ministry also referenced the SSP (Special Social Payment, ССВ) as an additional monthly benefit.
What this means: This is the region’s strongest financial argument. Most Far North territories expect young doctors to spend years slowly accumulating their northern bonus percentages. The Magadan model eliminates that wait entirely.
Question 5: Housing
Ministry’s response: Three housing programs are available. First, a social subsidy covering 30% of the calculated purchase price of housing within the Magadan Region, with a five-year mandatory service obligation from the date of the agreement. For 2025, the subsidy amounts to approximately ₽1,008,275 (~$10,083) for a single person, ₽1,283,260 (~$12,833) for a family of two, and ₽1,649,905 (~$16,499) for a family of three. Second, monthly rental compensation: in the municipal districts, ₽9,000 for a one-room apartment up to ₽14,000 for a three-room apartment; in Magadan city, ₽20,500 for a one-room apartment up to ₽26,700 for a three-room apartment. Third, service housing (служебное жильё) may be provided when vacancies exist in the regional housing stock.
What this means: The 30% subsidy figure requires context. The calculation is based on a «normative» cost per square meter set by the government, which is consistently lower than market prices. The practical coverage is discussed in Part 2 below.
Question 6: Internship and Travel Support
Ministry’s response: Contract students have their travel reimbursed for trips between their place of study and their internship site, and separately for annual holiday travel: from their place of study to their registered home address and back. Second-year ordinatura students employed as intern physicians during their practical training period also receive travel reimbursement to and from their assigned facility, as well as partial rental compensation while working.
What this means: Annual holiday flight reimbursement is uncommon across Russia’s regional contracts. For a student based in Moscow or another major city, this benefit has direct monetary value that compounds over six years of study.
Question 7: Choosing a Workplace
Ministry’s response: Contract training agreements are signed only for specialties in which the Ministry’s subordinate institutions have vacancies. For detailed guidance on applying, the Ministry directed inquiries to its Medical Personnel and Civil Service Department at +7 (4132) 62-92-11.
What this means: Since May 1, 2024, candidates select contract training offers through the «Work in Russia» portal (trudvsem.ru). The identity of the sponsoring organization (заказчик) determines what you can know in advance about your future workplace.
Question 8: Contract Terms and Termination
Ministry’s response: Workplace changes within the region are possible by mutual agreement when a vacancy exists. Contract termination is governed by federal legislation: Government Decree No. 555.
What this means: Federal law provides several grounds for penalty-free termination: establishment of a Group I or Group II disability, the need to care for a close relative with a Group I disability, or relocation required by a military spouse transferred to a new posting.
Part 2: What I Found Through Independent Research
Zemsky Doctor — The 50,000-Resident Cap in Practice
The entire Magadan Region qualifies for the maximum ₽2,000,000 Zemsky Doctor payment under Government Decree No. 1946: it is fully classified as Far North. The binding constraint is the population ceiling, not the geographic classification.
Magadan city has roughly 90,000 residents. Physicians assigned to the regional hospital, the oncology center, or the city’s outpatient clinics are automatically excluded from the program. The ₽2,000,000 payment is available only when assigned to district centers (Ola, Palatka, Yagodnoye) or small towns such as Susuman.
Settling-in Bonus — Reading the Logic
The regional payment scale rewards urban assignment more than rural, which appears paradoxical. A physician in Magadan city receives ₽300,000; one in a remote settlement receives ₽200,000. The reasoning is structural: rural physicians receive ₽2,000,000 from the federal Zemsky Doctor program, so the regional payment does not need to compensate as heavily. For the Magadan-assigned physician who loses the federal payment entirely, ₽300,000 is a modest consolation.
Real Income — What You’ll Actually Earn
The northern bonus elimination of the accumulation requirement is the most practically consequential feature of this contract. Under a standard Far North model, a physician with no prior Far North service would receive only their base salary of ₽35,817 while slowly earning bonus percentage points over years of service. Decree No. 535-pp, paragraph 8.7, requires instead that the payroll department apply the full 80% compensatory supplement immediately — in addition to the base salary. In the Severo-Evensky District, the supplement reaches 100%.
The same decree includes hazard pay supplements: from 4% for hazard class 3.1 up to 24% for work in HIV or psychiatric units.

A therapist in a district outpatient clinic can realistically expect a combined monthly income starting at ₽175,000 (~$1,750): base salary plus supplements of approximately ₽125,000, plus SSP of ₽50,000 (for settlements under 50,000 residents). The SSP (Special Social Payment) is a federal monthly supplement paid separately from salary. It is tax-free and excluded from average-earnings calculations. Physicians working in Magadan city (population above 50,000) receive the reduced SSP rate of ₽29,000/month.
One eligibility restriction applies: SSP is available only to primary care physicians, specifically general practitioners, pediatricians, and FAP feldshers. Narrow specialists employed in inpatient hospital settings do not receive it. An ophthalmologist or surgeon in a regional hospital should calculate income without the SSP line.
Housing — Subsidy vs. Market Reality
The 30% housing purchase subsidy sounds substantial. The catch is the calculation base. The government sets a «normative» price per square meter that consistently falls below actual market prices in Magadan. A one-room apartment on the open market costs between ₽4,500,000 and ₽6,000,000 (~$45,000–$60,000). The subsidy of approximately ₽1,008,275 covers 15–20% of that figure. The remaining ₽3.5–5 million must come from a mortgage, which at current Russian rates represents a serious financial obligation.
The rental compensation is more practical, though still partial.

Internship Travel — A Hidden Scholarship
Holiday flight reimbursement is a feature that does not appear in most regional contracts. A round-trip economy ticket between Moscow and Magadan costs between ₽60,000 and ₽100,000 (~$600–$1,000) in season. Across six years of study, this benefit saves a student’s family between ₽300,000 and ₽500,000 (~$3,000–$5,000), a substantial sum that makes central Russian medical education financially accessible for students from distant regions.
Workplace Selection — The Sponsoring Organization Question
From May 1, 2024, applicants select contract training offers on the «Work in Russia» portal (trudvsem.ru). The sponsoring organization named in the contract determines how much certainty you have about your future workplace. When the sponsoring organization is a specific CRH (the Olsky District Hospital, for example), the destination is fixed and you qualify for the full ₽2,000,000 Zemsky Doctor payment. When the sponsoring organization is the regional Ministry of Health, your actual workplace is determined after graduation. In that case, assignment to Magadan city, and exclusion from the Zemsky Doctor program, remains a possibility.
Contract — Transfers and Exit Conditions
Transfers between facilities within the Magadan Region are possible by mutual agreement when a vacancy exists. Government Decree No. 555 governs penalty-free contract termination at the federal level. The grounds include establishment of Group I or II disability in the contract student, the need to provide full-time care for a close family member with Group I disability, and mandatory relocation required when a military spouse receives orders to a new posting. These conditions apply uniformly across all regions.
Pros and Cons
The Magadan Region offers one of Russia’s more financially transparent contract packages, but the details require careful reading before signing.
The program’s clearest advantage is the immediate northern bonus. The legal guarantee in Decree No. 535-pp, paragraph 8.7, eliminates years of salary suppression that physicians in other Far North regions endure while accumulating service. From the first month, a physician in Magadan city receives an 80% compensatory supplement on top of base salary; in the Severo-Evensky District, the supplement is 100%. Combined with SSP of ₽50,000 for settlements under 50,000 residents, total monthly income from the start of employment reaches approximately ₽175,000. The Zemsky Doctor payment of ₽2,000,000 applies across all rural and small-town placements, and annual holiday flight reimbursement, covering ₽60,000–₽100,000 per round trip, adds between ₽300,000 and ₽500,000 in savings over the study period.
The program’s limitations are also concrete. Magadan city, the most comfortable and logistically convenient assignment, excludes physicians from the ₽2,000,000 Zemsky Doctor payment. The housing purchase subsidy covers only 15–20% of actual market prices, not the 30% the figure implies — because the calculation base is a government normative price, not the real market. Rental compensation is partial: in Magadan city, it covers 45–68% of actual rental costs depending on apartment size. Accepting multiple overlapping support measures (Zemsky Doctor, settling-in bonus, housing subsidy), each with its own mandatory service period, can create binding commitments that extend well beyond the five-year contract itself.
The decision to sign a contract in this region should rest on which district the placement will be in, whether the sponsoring organization is a specific hospital or the regional Ministry, and a realistic assessment of the mortgage burden if housing purchase is planned.
