₽2.5 Million for a Young Doctor in Bashkortostan: How to Stack Zemsky Doctor and the Regional Grant
The Republic of Bashkortostan pairs the federal Zemsky Doctor relocation bonus with a separate regional cash payment for deficit-specialty doctors, and the two can be collected simultaneously.
In 2024, the federal program alone paid out ₽144.5 million to 129 physicians in the Republic. This article is a detailed breakdown of the conditions for future physicians in Bashkortostan, prepared as part of the Navigator for Contract Students project.

Analysis of the Official Response
At the time of publication, the editorial team has not received an official response from the Ministry of Health of the Republic of Bashkortostan. The analysis below is based on open official sources, regulatory documents, and job market data.
1. Zemsky Doctor and Zemsky Feldsher
The Zemsky Doctor program pays a lump sum to physicians who relocate to rural areas or small towns. Payment amounts in Bashkortostan follow the standard federal scale: ₽1,000,000 (~$10,000) for doctors taking up positions in settlements with fewer than 50,000 residents, and ₽1,500,000 (~$15,000) for doctors posted to remote and hard-to-reach territories (the list is approved by the Government of the Republic of Bashkortostan). Feldshers receive ₽500,000 (~$5,000) and ₽750,000 (~$7,500) respectively under the Zemsky Feldsher program.
Bashkortostan’s districts are not classified as Far North or equivalent territories under Government Decree No. 1946, so the ₽2,000,000 rate does not apply here.
In 2024, 129 doctors received Zemsky Doctor payments totaling ₽144.5 million (~$1.45 million). The same budget — ₽144.5 million — has been allocated for 2025, with 61 doctors expected to receive ₽1.5 million each and 20 doctors receiving ₽1 million each. Under the Zemsky Feldsher program, 33 paramedical staff received payments in 2024 totaling ₽16.5 million.
To qualify, a doctor must hold Russian citizenship, be under 50 years old, hold a higher medical degree with a current accreditation certificate, and sign a labor contract for at least five years. Contract students (целевики) are eligible provided they have no outstanding unfulfilled obligations under their contract training agreement (целевой договор).
2. Settling-In Bonuses
Bashkortostan runs a separate regional program called «Physicians for Small Medical Organizations,» established under Government Decree of the Republic of Bashkortostan No. 285 of May 6, 2020. It pays ₽1,000,000 (~$10,000) to doctors who join Ministry of Health facilities serving a catchment population of fewer than 25,000 people. In 2024, 49 doctors received this payment; the same allocation — ₽49 million for 49 doctors — is planned for 2025.
The payment is tied to the vacancy, not the doctor’s general qualifications. The physician must fill a position listed on the «Register of Deficit Specialties,» which the regional Ministry of Health is required to update at least once a year. Historically, the list covers anesthesiologists-resuscitators, emergency physicians, psychiatrists, and district GPs and pediatricians in remote districts.
The regional regulations contain no prohibition on combining this payment with the Zemsky Doctor bonus. A doctor who takes a deficit-specialty position in a remote territory can therefore claim ₽1,500,000 (Zemsky Doctor) + ₽1,000,000 (regional program) = ₽2,500,000 (~$25,000) in upfront capital.
In 2023, regional authorities discussed extending the program to allow contract students to receive up to ₽3 million total. No evidence that this amendment was adopted has been found in open sources.
3. Base Salary
Physician compensation at state health facilities in Bashkortostan is governed by Government Decree of the Republic of Bashkortostan No. 311 of August 13, 2015 (as amended June 11, 2024). Base salaries are calculated by multiplying a base unit of ₽4,885 by a professional qualification group (PKG) coefficient.

A doctor’s take-home pay has three layers. The first is the base salary shown above. The second layer is compensatory payments (компенсационные выплаты): a regional multiplier of 1.15 (15%) applies across the entire Republic, with additional bonuses for hazardous conditions — working with HIV patients, tuberculosis, or in psychiatry — and enhanced pay for night shifts. The third layer is incentive payments (стимулирующие выплаты): a point-based system that rewards performance, seniority, and qualification category, plus «May Decree» bonuses to bring salaries toward 200% of the regional average.
The Republic’s minimum wage since January 1, 2025, including the regional multiplier, stands at ₽27,277.
4. Real Income for a Young Doctor
According to the trade union of healthcare workers of the Russian Federation, the average salary for medical staff in Bashkortostan in the first half of 2023 was ₽49,200. Doctors specifically averaged ₽84,200 per month; nursing staff averaged ₽46,600.
Incentive payments vary sharply depending on the hospital’s finances — from around ₽10,000 in underfunded rural hospitals to ₽100,000 in well-resourced urban institutions.
Special Social Payment (SSP)
Under Government Decree No. 2568 (PP RF No. 2568), primary care physicians in Bashkortostan receive a monthly federal SSP — Special Social Payment on top of their regular salary. In settlements with fewer than 50,000 residents, the SSP is ₽50,000 per month. In settlements of 50,000–100,000 residents, it is ₽29,000 per month. The SSP does not apply in cities over 100,000, and narrow inpatient specialists do not receive it regardless of location.

For a district GP in a small town under 50,000 residents, the combined income works out as follows: a base salary with regional multiplier and incentive payments of roughly ₽60,000, plus an SSP of ₽50,000, gives a total monthly income of ₽110,000 (~$1,100). For the same doctor in a town of 50,000–100,000 residents, the base rises toward ₽70,000 and the SSP drops to ₽29,000, for a combined total of ₽99,000 (~$990).
The average salary across Bashkortostan was ₽67,576 in 2024 and ₽74,377 as of July 2025 (Rosstat data). Starting vacancies for new doctors without SSP sit at ₽28,000–₽60,000, or 41–88% of the regional average. Once SSP is factored in, a district GP in a small town earns 1.3–1.5 times the regional average.
Career trajectory
After completing the mandatory service period (отработка) and, if applicable, ordinatura, the path to narrow specialties opens. According to BDEX salary data, the median in Bashkortostan medicine spans ₽29,830 to ₽85,930. A chief physician earns around ₽85,930, a head of department roughly ₽64,560, a surgeon approximately ₽55,210.
5. Housing
Federal preferential mortgage programs. Physicians in Bashkortostan can access federal subsidized mortgage schemes. Several major banks offer mortgage rates starting at 4.7% for families with two or more children, or 5.2% for families with one child. DOM.RF Bank offers a rate reduction to medical workers in official employment at healthcare facilities; standard market rates currently range from 21% to 29.4%.
Regional Housing Savings Program (HSP). Bashkortostan operates a housing savings scheme through partner banks, primarily Sberbank and Uralsib. A participant opens a dedicated savings account for three to six years. The Republic’s budget adds a 30% bonus on top of contributions, up to ₽5,000 per month. On completion of the savings period, the participant gains access to a mortgage at a preferential rate of 5.5–7% annually.
A practical example: a contract student opens the account in their third year and contributes ₽3,000 per month. By graduation four years later, they have accumulated an initial down payment boosted by 30% in budget premiums, plus eligibility for a below-market mortgage at 6–7%. Given that current commercial mortgage rates stand at 21–29%, this represents a substantial saving for anyone planning to purchase property.
Other regional measures. In 2022, the Republic provided housing to 186 physicians and 43 nursing staff members. Additional options mentioned in regional reports include participation in a «Social Mortgage» program, rental reimbursement, service housing, and allocation of land plots to physicians. The specific conditions and regulatory documents governing these arrangements at the regional level were not available in open sources at the time of writing.
6. Support During Internships
The internal internship regulations of the Bashkir State Medical University (BSMU) set the following allowances for contract students traveling outside Ufa for practical training: a per diem of ₽50 per day and housing reimbursement of up to ₽550 per day. Travel within the Republic is not reimbursed.

Over four mandatory internships, the total out-of-pocket cost can exceed ₽160,000 (~$1,600).
The BSMU regulations include one important caveat: students formally employed at a healthcare facility fall under the Labor Code of the Russian Federation. A practical workaround is to contact the personnel department of the target hospital two or three months before the internship and arrange to be employed as junior or nursing staff — a sanitary worker, or a nurse following completion of the third year and the relevant clearance. A month’s salary in that role runs ₽20,000–₽30,000, turning a ₽40,000 loss into a net income.
7. Choosing a Workplace
Information on how contract student placement works is published on the official website of the Ministry of Health of the Republic of Bashkortostan. Under the regional system, the contract training agreement is concluded between the student, the Ministry of Health of Bashkortostan, and the specific healthcare facility under the Ministry’s jurisdiction.
To initiate the process, the student submits an application addressed to the Minister of Health specifying the desired specialty, accompanied by a supporting letter from the target medical organization. The student selects the organization independently before the agreement is signed.
Under the new federal procedure in force since May 1, 2024, applicants select their place of work through the «Work in Russia» portal (trudvsem.ru) at the time of application. When reviewing available offers, the student should pay attention to who the sponsoring organization (заказчик) is. If it is a specific hospital, the workplace is defined from the start. If it is the regional Ministry of Health as a whole, the actual facility will be determined later, and that point should be clarified before signing.
8. Contract Terms: Transfers and Termination
Changing the workplace. No detailed regional regulations governing the procedure for transferring a contract student to a different facility were found. Under federal legislation, the place of employment is stated in the contract training agreement and may be changed by mutual consent of all parties through a supplementary agreement.
Terminating the contract. Grounds for penalty-free termination of a contract training agreement are established by Government Decree No. 555 of April 27, 2024. The contract can be dissolved without financial liability for the student in three main scenarios: if the student is assigned a Group I or II disability that prevents continued study or employment; if the student must provide permanent care for a sick family member as confirmed by a medical institution; or if the student’s spouse, who serves in the military, is transferred to a different region.
If an employer terminates the labor contract on grounds provided by the Russian Labor Code, the contract training agreement is considered simultaneously dissolved, with neither party bearing financial responsibility. If the sponsoring organization fails to provide the agreed social support measures for six months from the date they were due, the student has the right to unilaterally terminate the agreement without penalty.
Student protection during the study period. A contract training agreement must include a commitment by the sponsoring organization to provide social support during the study period — typically a supplemental stipend of ₽1,000–₽2,000 per month. If the hospital delays or stops these payments for six months, this constitutes a material breach on the employer’s side. Court practice shows that in such cases the agreement can be dissolved and the student released from any obligation to reimburse tuition costs.
Conclusions
Reasons to consider Bashkortostan
The regional «Physicians for Small Medical Organizations» program pays ₽1,000,000 on top of whatever federal bonuses apply, specifically for doctors entering facilities serving fewer than 25,000 people. In 2024, 49 doctors received this payment. Stacking it with the Zemsky Doctor bonus for a remote posting yields ₽2,500,000 (~$25,000) in upfront capital — a combination not available in most Russian regions.
The Republic’s participation in federal programs is consistent: 129 doctors received Zemsky Doctor payments in 2024. The housing savings program, with its 30% government match and access to a 5.5–7% mortgage, is a concrete advantage at a time when market rates exceed 20%.
For primary care doctors in small towns, combined income including SSP reaches ₽99,000–₽110,000 per month, which is 1.3–1.5 times the regional average wage.
Reasons to think carefully
Starting salaries without SSP range from ₽28,000 to ₽60,000 — 41–88% of the regional average. The base salary for a district GP sits at ₽28,857 (₽33,186 with the regional multiplier); the gap to a liveable wage is filled by incentive payments whose size depends on the hospital’s budget and the chief physician’s discretion.
The SSP is only available to primary care physicians. Narrow specialists in inpatient facilities receive no federal monthly supplement, which makes their income trajectory substantially different from that of GPs and pediatricians.
The internship allowances are nominal — ₽50 per day in per diem and ₽550 per day in housing reimbursement cover a small fraction of actual costs. Without proactive action to arrange paid employment during the internship, a student can lose over ₽160,000 across four mandatory placements.
