Contract Training in Chuvashia: Starting Pay ₽60,000–75,000 and a ₽1M Regional Bonus the Ministry Forgot to Mention

Part 1: What the Ministry of Health Told Me
On August 15, 2025, the Ministry of Health of the Chuvash Republic responded to a formal written inquiry submitted on July 24, 2025. The response was signed by Deputy Minister Alina V. Petrova and carried reference number 01/13-12555.
Question 1: Zemsky Doctor eligibility
Ministry’s response: Contract students (целевики) are eligible for the Zemsky Doctor program. The Ministry cited Government Decree No. 954 of July 13, 2024, which extended the program to graduates who return to their place of residence — a rural settlement, working settlement, urban-type settlement, or a city with a population under 50,000.
What this means: The ministry’s answer is technically accurate but incomplete. The standard Zemsky Doctor payment for Chuvashia’s rural and small-city locations is ₽1,000,000 (~$10,000) for physicians. The ministry did not state this figure explicitly, which leaves applicants to find the amount on their own.
Question 2: Settling-in bonus (подъёмные)
Ministry’s response: A one-time payment of ₽200,000 (~$2,000) is available to primary care physicians. The Ministry added that individual hospitals may establish additional one-time payments through their own local regulations.
What this means: The number is real but applies only to GPs, pediatricians, and general practitioners under 35 who are hired by city hospitals. The payment is not accessible on the first day of work — the right to it arises after one year of employment and must be exercised within three years. More critically, the official response says nothing about a separate regional program worth ₽1,000,000 that was approved two months before this letter was written.
Question 3: Base salary
Ministry’s response: The base salary (должностной оклад) for a specialist physician ranges from ₽27,882 to ₽36,372. The figure is set by Decree of the Cabinet of Ministers of the Chuvash Republic No. 451 of December 14, 2015, with annual indexation. Most recently, salaries were indexed by 6% in January 2023 and by 8.1% in January 2025.
What this means: The numbers are accurate and current. The underlying document dates to 2015, but repeated indexations have brought the figures to their present level. The base salary is the foundation — total take-home pay is higher once compensatory and incentive payments are added.
Question 4: Real income
Ministry’s response: In 2024, the average monthly wage for medical personnel across the healthcare sector as a whole was ₽90,241.8. By institution type: republican hospitals — ₽100,742.1; Cheboksary city facilities — ₽90,213.7; central district hospitals (CRH) — ₽74,363.0.
What this means: The averages are published data, but they blend experienced specialists, surgeons, department heads, and heavy overtime into a single figure. A newly graduated physician without an established bonus record will land considerably below these numbers.
Question 5: Housing
Ministry’s response: The ministry listed eight support mechanisms, including priority lease of public housing, service housing through local authorities, mortgage interest subsidy reducing the rate to 1% per annum, a social grant covering up to 70% of housing cost in rural areas, a monthly utility compensation of ₽1,377 for rural medical workers, and reimbursement of commuting costs.
What this means: The list is long, but most items are either conditional mortgage instruments requiring years of prior employment or depend on local authority allocations that vary by municipality. The only concrete and universally applicable figure is the ₽1,377 monthly utility payment for rural workers.
Question 6: Internship support
Ministry’s response: Internships are conducted at the medical organization named in the contract training agreement (целевой договор). Decrees No. 254 and No. 255 of the Cabinet of Ministers of the Chuvash Republic (May 8, 2024) do establish rent and travel reimbursement programs — but explicitly only for employees with active work contracts, not for students during placement. The Ministry stated plainly: these measures are not available to contract students during their internship periods.
What this means: Students studying outside Chuvashia bear all travel and accommodation costs personally.
Question 7: Choosing a workplace
Ministry’s response: Since May 1, 2024, contract training offers are published on the «Work in Russia» portal (trudvsem.ru) with a specific employer identified for each position.
What this means: The portal system provides clarity — but only when the offer specifies a hospital. If the sponsoring organization (заказчик) listed is the Ministry of Health itself rather than a named facility, the actual posting may not be determined until close to graduation.
Question 8: Contract modification and termination
Ministry’s response: Changes to the contract — including a change of workplace — require agreement from all parties and are formalized through a supplementary agreement. Under Government Decree No. 555 of April 27, 2024, a student may terminate the contract unilaterally without penalty if the agreed support measures are not provided within six months of the due date.
What this means: «By agreement of parties» gives the employer full discretion to refuse a transfer. The student has no independent right to demand reassignment. Federal grounds for penalty-free termination are specific and include Group I or II disability assigned to the student or a close relative (spouse, parent, or child) that prevents living or working in the assigned location, the need for permanent care of a Group I disabled immediate family member, and relocation of a military spouse to a new posting.
Part 2: What I Found Through Independent Research
Zemsky Doctor — Eligibility and the Sequencing Problem
Chuvashia is not classified as the Far North or a territory equated to it. The standard Zemsky Doctor payment for physicians working in rural areas and towns under 50,000 residents is ₽1,000,000 (~$10,000). The July 2024 amendment allowing graduates to claim the payment in their home locality is a practical improvement — it removes an earlier restriction that made the program inaccessible to returning locals.
The timeline is where careful reading matters. Zemsky Doctor is not a parallel benefit during the mandatory service period (отработка). It is a separate, subsequent program. A typical sequence: three to five years under the contract training agreement, then — if the graduate chooses to sign a Zemsky Doctor contract at the same location — another five years. Total time in the region before collecting all benefits: eight to ten years.
Regional Settling-in Bonus — the ₽1,000,000 Program the Ministry Did Not Mention
The official response describes the ₽200,000 payment for urban primary care physicians. What it does not mention is Decree of the Cabinet of Ministers of the Chuvash Republic No. 257 of May 8, 2024 — a regional program paying ₽1,000,000 (~$10,000) to physicians with higher medical education who take up positions in rural areas and small towns. The payment is staggered: one installment in year one, another in year three, and a final installment in year five.
A news report from the Cheboksary portal «Pro Gorod» published after the decree’s adoption announced that «young specialists will receive a million rubles — but only if they leave the capital.» The contrast is telling: the policy exists, was publicly reported, and was enacted two months before the Ministry wrote its response. An applicant relying solely on the official letter would never know about it.
Base Salary — What the Numbers Actually Cover
The range of ₽27,882–₽36,372 represents the base salary before incentive and compensatory payments. Total take-home pay is higher. Compensatory payments cover night shifts, weekend duties, overtime, and hazardous conditions. Incentive payments reflect workload intensity, quality scores, and quarterly or annual bonuses. Neither category has a fixed floor — amounts depend on the individual institution’s budget and the employee’s actual schedule.
Real Income — SSP and What Vacancies Show
The ministry’s ₽90,241 average is a sector-wide figure that reflects the full distribution of physician incomes, including senior specialists with decades of experience and heavy caseloads. A new graduate entering on a standard schedule in a district facility will see a different number.
Vacancy analysis from hh.ru provides a grounded baseline. District GPs and pediatricians without prior experience are offered ₽60,000–₽75,000 (~$600–$750) total monthly income in Cheboksary. After three to five years — roughly the length of a contract training service period — the same position pays ₽85,000–₽110,000 (~$850–$1,100).

The Ministry’s response does not mention the SSP — Special Social Payment (специальная социальная выплата). This is a federal monthly supplement, tax-free and excluded from average earnings calculations. GPs, pediatricians, and general practitioners working in localities under 50,000 residents receive ₽50,000 (~$500) per month. In towns of 50,000–100,000 residents, the payment is ₽29,000 (~$290) per month. The supplement applies only to primary care physicians and FAP feldshers — narrow specialists in inpatient facilities are not eligible.
For a district GP in a rural posting or town under 50,000, the combined monthly income would be approximately ₽75,000 (net base + incentive) plus ₽50,000 SSP, reaching around ₽125,000 (~$1,250) total. That figure looks very different from the ₽60,000–75,000 advertised on vacancy boards, and it also exceeds the ministry’s published average.
Housing — What the List of Programs Means in Practice
Eight programs listed, one concrete number: ₽1,377 per month in utility compensation for rural medical workers. Everything else requires either an active mortgage, a multi-year employment record, or a place in a municipal waiting list.

The May 2024 decrees do announce rent reimbursement for relocated medical workers — but only once they hold a signed employment contract. During training, there is no support. After employment begins, the practical coverage depends on municipal implementation, which varies across Chuvashia’s 21 districts.
Internship Costs — the Hidden Expense
Students who study in universities outside Chuvashia — the most common choice being Kazan or Moscow — must travel to the republic for each placement and cover their own costs. Four placements over the course of medical education is a standard requirement.

Choosing a Workplace — One Detail That Changes Everything
The federal procedure established by Government Decree No. 555 requires that each offer on the «Work in Russia» portal (trudvsem.ru) name a specific employer. For applicants, this distinction is the most consequential part of reading an offer carefully: if the sponsoring organization (заказчик) is a named hospital, the placement is fixed at the point of signing. If the sponsoring organization is listed as the Ministry of Health of the Chuvash Republic, the specific facility will be determined later — potentially in the final year of study — based on the republic’s staffing needs at that time.
Contract Terms — Transfer and Exit
Changing the assigned workplace requires written consent from all parties to the agreement. The employer has no obligation to agree. No regional regulation was found that limits the employer’s discretion or sets a procedure for resolving a deadlock.
Grounds for terminating the contract without financial penalty exist under federal law. Government Decree No. 555 covers cases where the student or an immediate family member is assigned Group I or II disability that prevents living or working at the assigned location, where a close relative requires permanent personal care at Group I disability level, and where a military spouse receives orders to a new posting.
Pros and Cons
Contract training in Chuvashia offers a genuine package of financial support, but the information provided through official channels does not reflect the full picture — which creates a risk of misinformed decisions.
The strongest argument in favor is the ₽1,000,000 regional bonus for rural physicians under Decree No. 257. Combined with the Zemsky Doctor payment of ₽1,000,000 and SSP of ₽50,000/month, rural postings become financially competitive in ways that are not apparent from the official letter alone. The starting salary of ₽60,000–₽75,000 in Cheboksary is respectable for a new graduate, and the three-to-five-year growth trajectory to ₽85,000–₽110,000 aligns with the national picture for this specialty. The portal-based selection system offers genuine transparency when the sponsoring organization is a specific hospital rather than the ministry.
The drawbacks cluster around information quality and housing. The official response omits the ₽1,000,000 bonus program entirely — an applicant who bases their decision on that letter will evaluate the offer against a ₽200,000 ceiling rather than a ₽1,000,000 ceiling. Internship costs are real and fall wholly on the student: ₽15,600 per trip adds up over a medical degree. Housing support in Cheboksary amounts to zero direct rent assistance, and the rural utility subsidy of ₽1,377 covers a negligible share of market costs. The ₽90,241 average salary figure, while accurate as a statistical aggregate, is likely to create unrealistic income expectations in a new graduate.
The decision to sign a contract training agreement in Chuvashia should be made with both the ₽1,000,000 rural bonus and the SSP supplement clearly factored in — not on the basis of the official response alone.
