Ivanovo Region: A Physician’s Base Salary Is ₽16,609 — Can a ₽900K Mortgage Bonus Make It Work?

The Ivanovo Region ranks last among all regions of the Central Federal District by average wage, at approximately ₽57,000, according to Rosstat data from autumn 2025.
Below, we examine whether regional support measures offset that record-low base income, and what non-obvious risks are embedded in a contract with the Regional Department of Health.

1. Zemsky Doctor and Zemsky Feldsher
Federal programs remain active in the region through 2030, at standard national payment rates.

Two conditions catch graduates off guard. First, the staffing threshold: the program only applies to facilities where staffing levels fall below 60%. At institutions where the shortage is less acute, the payment becomes inaccessible until the mandatory service period (отработка) ends. Second, the geography: a graduate assigned to the city of Ivanovo, with a population of approximately 400,000, receives nothing under this program. Zemsky Doctor covers only settlements under 50,000 residents.
2. Settling-in Bonuses and Regional Payments
The Ivanovo Region offers a two-tier regional support system on top of federal programs.
Young physicians who start work immediately after graduation receive a one-time settling-in bonus (подъёмные) of ₽300,000 (~$3,000). Emergency feldshers qualify for a separate one-time payment of ₽200,000 (~$2,000).
The mortgage subsidy is a separate item. Since 2025, physicians under 40 can receive ₽900,000 (~$9,000) toward a mortgage down payment. The catch is contractual: accepting this payment triggers a five-year work obligation on top of the standard contract training commitment. A contract student (целевик) who accepts the mortgage payment effectively enters a double-obligation structure: first the training contract, then the mortgage term, which may run concurrently or extend beyond it depending on timing.
3. Base Salary
During research, Appendix 1 to the Regulation on Remuneration (amended by Decree of the Government of the Ivanovo Region No. 404-p, dated October 15, 2025) was located. The figures confirm a critical structural problem with physician compensation in the region.

The guaranteed portion of a physician’s pay represents only about 25% of the regional average wage (₽68,600). The remaining 75% comes from incentive payments (bonuses and seniority allowances) that depend entirely on hospital management decisions and available budget. Any conflict with administration puts the specialist in a precarious position.
4. Real Income: Salary Plus SSP
Low base salaries in the region are partially offset by the federal SSP (Special Social Payment, ССВ). Since 2024, the SSP amount has varied sharply by settlement size, creating a concrete financial incentive to work in small towns rather than the regional capital.

The SSP is tax-free and excluded from average-earnings calculations. For a general practitioner in a district hospital, the combined income picture looks as follows.

A primary care physician in a small town earns roughly 1.8 times the regional average wage once SSP is included. Narrow specialists working in inpatient facilities (surgeons, cardiologists, neurologists) do not receive SSP at all, which produces a stark income gap between primary care and hospital medicine.
5. Housing: Costs and Availability
No formal regional rent compensation program was identified in open sources. Individual facilities (Furmanov CRH is one example) mention housing support in their job listings, but no region-wide program exists.
The rental market itself partly compensates for low wages. Costs across the region are among the lowest in the Central Federal District.

The regional average rental cost sits at approximately ₽14,600 per month, low enough that a physician receiving SSP retains a reasonable disposable income even without institutional housing support.
6. Practical Training Costs
No regional programs covering travel or accommodation for interns during mandatory placements were identified. All expenses fall on the student.

Over the full course of study, four mandatory placements amount to approximately ₽60,000–₽70,000, a cost that adds up quickly for families financing their student’s education.
7. Choosing a Workplace
State assignment (распределение) has operated through the «Work in Russia» portal (trudvsem.ru) since May 1, 2024. When selecting an offer, the type of sponsoring organization (заказчик) is the single most consequential detail in the contract.
When the sponsoring organization is a specific hospital (Kineshma CRH, for example), the graduate knows from day one exactly where they will work. The placement is legally fixed at the time of signing.
When the sponsoring organization is the Regional Department of Health, the department acts as an aggregator and assigns graduates upon graduation «based on regional staffing needs.» The actual workplace is unknown at the time of signing. In practice, the highest demand is in the most understaffed facilities, which tend to be in remote or economically depressed districts. Reading the contract carefully before signing is not optional.
8. Contract Terms: Modification and Termination
Government Decree No. 555 of April 27, 2024 governs the federal framework for contract training agreements (целевой договор). Under this decree, a contract can be dissolved without financial penalties under four circumstances: establishment of a Group I or II disability; the need to provide care for a close relative with Group I disability; relocation following a military spouse’s reassignment; or confirmed inability to work in the specialty based on a medical report.
Outside these grounds, termination triggers a penalty equal to all social support received plus the cost of the education itself. Over six years of medical school, that penalty can reach ₽1,000,000 (~$10,000). Workplace transfers within the region are governed by a separate regional mechanism and depend on the sponsoring organization’s consent.
Pros and Cons
Contract training in Ivanovo presents a sharply divided picture: some of the most generous lump-sum payments in the Central Federal District sit alongside the region’s worst base salary figures.
The financial case for signing is built on three pillars. The mortgage subsidy of ₽900,000 and the ₽300,000 one-time bonus together represent a combined starting package of ₽1,200,000, a level of upfront support that few neighboring regions match. For a GP working in a small town, SSP alone adds ₽50,000 per month on top of regular wages, pushing total monthly income to around ₽100,000 despite the low base. Rental costs in the region start at ₽7,000 per month, which means the cost of living is genuinely manageable on a rural physician’s income. The four-hour train connection to Moscow also reduces the sense of professional isolation.
The case against rests on structural risks. The officially approved base salary of ₽16,609 is not a formality — it is the actual floor a physician earns if bonuses are withheld, and it is legally enforceable as such. Getting the Zemsky Doctor payment requires working at a facility with under 60% staffing, which in practice means the most understaffed and overworked hospitals. Narrow specialists in inpatient settings receive no SSP and no Zemsky payment if assigned to Ivanovo city, producing a combined monthly income that may not exceed ₽50,000. Internship travel costs are entirely out-of-pocket, and the mortgage subsidy creates a second contractual obligation that extends the total commitment timeline.
The decision warrants careful calculation before signing, particularly for anyone considering hospital specialty tracks or who may be assigned to the regional capital.
