Contract Training in Penza: Physicians Earn Up to ₽140,000 — But the Ministry Won’t Tell You How

Part 1: What the Ministry of Health Told Me

The Ministry of Health of Penza Region replied to my written inquiry with a brief letter. Across all eight core questions (Zemsky Doctor payments, settling-in bonuses (подъёмные), internship support, housing programs, and contract terms), the department provided no figures, no procedures, and no specifics. The official position was that students receive this information orally at general orientation meetings.

When the Ministry cross-checked enrollment lists and did not find my name, it declined to share data at all. The episode reflects the system’s fundamental access problem: a prospective applicant cannot obtain reliable information before actually enrolling. The formal channel is closed until you are already inside.

Since official communication produced nothing usable, everything that follows comes from regulatory documents, federal statistics, job portals, and real estate data.


Part 2: What I Found Through Independent Research

Zemsky Doctor and Zemsky Feldsher

The standard federal program rates apply across Penza Region. Physicians who relocate to rural settlements, workers’ villages, or cities with fewer than 50,000 residents receive a one-time payment of ₽1,000,000 (~$10,000). Feldshers under the same conditions receive ₽500,000 (~$5,000). In remote and hard-to-reach territories (the qualifying list is approved annually and in Penza Region covers specific villages in Zemetchinsky District), the rates rise to ₽1,500,000 (~$15,000) for physicians and ₽750,000 (~$7,500) for feldshers.

The program runs until 2030. Participants commit to a five-year mandatory service period (отработка).

One structural hazard: the payment is tied to a vacancy registered in the program registry. If the hospital is considered adequately staffed at the time of graduation (typically defined as above 60% occupancy), the entitlement lapses, even if the locality meets the geographic criteria. This is the «staffing trap,» and it is not mentioned in most orientation materials.

Settling-in Bonuses

No information on large regional settling-in bonuses (such as the ₽200,000–₽500,000 lump sums offered in neighboring oblasts) appeared in any public source. The region’s strategy for attracting physicians relies on other mechanisms.

From September 2024, contract students (целевики) from Penza Region receive an additional scholarship of ₽1,000 per month from the regional budget during their studies.

Base Salary

The compensation framework for regional medical institutions is set by Resolution of the Government of Penza Region No. 303-pP of June 5, 2018. The base salary for a specialist physician ranges from ₽21,000 to ₽23,000 per month. Everything beyond that base comes from incentive payments, which vary by facility-level efficiency metrics and management discretion.

Real Income — What You’ll Actually Earn

A low base salary is standard across Russian public medicine; the actual take-home figure looks quite different. Federal supplements and on-call shifts push earnings well above the base. According to the Territorial Compulsory Medical Insurance Fund, the average physician salary in 2024 was ₽90,627 (~$906), rising to ₽95,000–₽96,000 (~$950–$960) in 2025. Both figures exceed neighboring Mordovia (₽71,800) and Ulyanovsk Region (₽74,500).

The Special Social Payment (SSP) adds further income, tax-free and paid monthly:

SSP applies to primary care physicians: GPs, pediatricians, general practitioners, and FAP feldshers. Narrow specialists in inpatient facilities do not receive it.

A GP working in a rural district, drawing an average salary of ₽90,000 (including on-call shifts) plus the maximum SSP of ₽50,000, reaches a combined ₽140,000 per month (~$1,400). At that level, a rural posting becomes financially more attractive than working in Penza city itself.

Housing Programs

Since 2021, the region has transferred 126 service housing apartments to medical institutions and purchased another 96 units for young specialists, distributed across all districts.

Rental compensation is also offered, though the exact terms are undisclosed in public sources. Based on comparable programs elsewhere, typical amounts fall in the ₽10,000–₽15,000 per month range. How far that goes depends entirely on location:

In district centers such as Serdobsk or Kamenka, rents run ₽8,000–₽12,000, so the compensation can cover the full cost. In Penza city, a physician renting at market rate will pay the difference out of pocket.

Internship Support

No public document records any travel or accommodation compensation for mandatory internships. Standard contract terms in Russian medical education place these expenses on the student.

Four mandatory internships over the study period bring the estimated hidden cost of a contract to ₽75,000–₽100,000 (~$750–$1,000) — an amount that appears nowhere in the official contract paperwork.

Choosing a Workplace

Under Government Decree No. 555, effective May 1, 2024, applicants select contract training offers through the «Work in Russia» portal (trudvsem.ru). The decisive factor is who acts as the sponsoring organization (заказчик).

When the sponsoring organization is a specific hospital, the future workplace is defined at the point of signing. When it is the regional Ministry of Health, the actual placement happens after graduation, based on staffing needs across the oblast at that time. In practice, the graduate can be sent to any district.

Contract Terms

No publicly available regional acts govern the procedure for changing a posting after graduation. Any transfer requires a supplementary agreement signed by all parties. Penalty-free termination is governed by federal law and available in a narrow set of circumstances: if a physician is assigned a Group I or Group II disability; if they must care for a disabled relative and no other caregiver exists; or if a military spouse is transferred to a new duty station.


Pros and Cons

Contract training in Penza Region offers a financially coherent path, especially for physicians prepared to work in rural areas, but administrative opacity and distribution uncertainty make careful due diligence essential.

The financial case for a rural posting is solid. A GP combining average earnings with the full SSP reaches ₽140,000 per month, above the regional average and ahead of Mordovia and Ulyanovsk. The standard Zemsky Doctor payment of ₽1,000,000 provides a lump sum at the start of employment. Service housing has been actively acquired since 2021, and district rental markets are affordable enough that compensation often covers the full monthly cost.

The drawbacks concentrate in transparency and flexibility. The regional Ministry of Health provided no usable information to a prospective applicant before enrollment, confirming that reliable data is difficult to access until you are already in the system. Contracts signed with the Ministry, rather than a specific hospital, leave the actual workplace undefined until graduation. Internship travel and accommodation costs fall entirely on the student, adding an estimated ₽75,000–₽100,000 over the study period. In Penza city, the SSP does not apply and rental compensation falls short of market rates, making a city-based placement less competitive financially.

The contract warrants careful reading before signing — particularly the identity of the sponsoring organization.
