Contract Training in Kursk Oblast: ₽19,500 Base Salary, ₽78,000 Rural Package, No Regional Settling-In Bonus

Part 1: What the Ministry of Health Told Me
The Ministry of Health of Kursk Oblast responded to my inquiry in full. Below is a summary of each answer, followed by what the response actually means for a prospective contract student (целевик).
Question 1: Zemsky Doctor
Ministry’s response: The Zemsky Doctor program is available to medical workers who have fulfilled their financial obligations under a contract training agreement (целевой договор). The program covers rural settlements, workers’ settlements, and cities with populations under 50,000. Timelines and payment amounts follow federal regulations.
What this means: The phrasing «fulfilled financial obligations» is the critical clause. The state avoids funding the same person twice — once by paying for six years of education, and again with a lump-sum relocation payment for working at the same facility. In practice, contract students can access the Zemsky Doctor program only after completing their mandatory service period (отработка). This is not a ban — it is a delay.
Question 2: Settling-In Bonus
Ministry’s response: No regional lump-sum settling-in bonus (подъёмные) exists for newly hired physicians. The Ministry pointed instead to the federal SSP — Special Social Payment (ССВ) — which can reach ₽50,000 per month for physicians in settlements under 50,000 residents.
What this means: There is no one-time regional cash payment to cover moving costs or the first month’s rent. The SSP is a monthly supplement, not a settling-in grant, and does not solve the immediate cash-flow problem of relocation. Graduates arriving in Kursk Oblast without savings will need to bridge that gap themselves.
Question 3: Base Salary
Ministry’s response: Salary amounts are set by Government Decree of Kursk Oblast No. 277-pp of April 9, 2024. The stated average physician salary for the elapsed period of 2025 is ₽92,048.
What this means: The ₽92,048 figure represents an average that includes experienced physicians with specialty categories, night-shift bonuses, and multiple rates of pay. For a newly graduated physician without a category or accumulated service, the relevant number is the base salary (должностной оклад) established in the decree’s appendix — not the average.
Question 4: Real Income and SSP
Ministry’s response: Salary consists of three components — base salary, compensatory payments (for night shifts, hazardous conditions), and incentive payments (for service length, intensity, specialty category). The average is ₽92,048.
What this means: A first-year physician without a qualification category receives none of the incentive bonuses tied to experience. The realistic starting income is substantially below the reported average.
Question 5: Housing
Ministry’s response: Three programs exist: interest compensation on mortgage loans (Decree No. 297-pa of March 30, 2012); compensation of rent and utilities for physicians working and living in rural settlements (Decree No. 419-pa of April 12, 2022); and provision of land plots for individual housing construction under Law No. 33-ZKO of June 2, 2020.
What this means: Each program has conditions that limit its practical reach. The rental compensation applies only to rural settlements — physicians placed in district-level cities such as Zheleznogorsk or Kurchatov are excluded. Land plots require construction financing that most new graduates do not have. The mortgage interest subsidy is functional but presupposes a down payment.
Question 6: Practical Training
Ministry’s response: Practical training conditions are set by the educational institution, not the Ministry. During studies, students receive a monthly payment not lower than the state academic scholarship, governed by Decree No. 569-pp of July 17, 2024.
What this means: Travel and accommodation during internships are not reimbursed. Students placed at distant facilities bear those costs themselves.
Question 7: Choice of Workplace
Ministry’s response: The specific workplace is determined at the time of graduation, based on the needs of facilities subordinate to the Ministry of Health of Kursk Oblast. The Ministry itself is the sponsoring organization (заказчик).
What this means: At the time of signing the contract, the applicant does not know which hospital or clinic they will be assigned to. The sponsoring organization is the regional ministry as a whole — not a specific facility. The assignment comes later, when the ministry assesses staffing needs at graduation.
Question 8: Contract Terms
Ministry’s response: Contract formation and termination follow Federal Law No. 273-FZ and Government Decree No. 555 of April 27, 2024. No regional exceptions were mentioned.
What this means: Penalty-free termination is possible only under the grounds listed in the federal decree. These are narrow. Regional specifics — such as whether the ministry allows internal transfers between facilities — were not addressed in the response.
Part 2: What I Found Through Independent Research
Zemsky Doctor: Amounts and Eligibility
Kursk Oblast is not classified as Far North territory, nor does it fall under the DFO or Arctic zone. Standard rates under Government Decree No. 1640 apply.

The delay built into the Ministry’s eligibility wording is a structural feature of the program, not an exception. The state pays for medical school through the contract quota, then pays a recruitment bonus only after the graduate has completed the mandatory service period. Both payments come from public funds, and federal rules prevent them from being claimed simultaneously.
Base Salary Under the Tariff Grid
Government Decree No. 277-pp of April 9, 2024, establishes fixed base salaries for healthcare workers in Kursk Oblast’s regional institutions.

The ₽19,500 base salary is all that is contractually guaranteed. Everything else — the compensatory and incentive components that bring the average to ₽92,048 — depends on shift load, specialty category, and facility-level decisions. A new graduate has none of the experience-based bonuses and no specialty category yet.
Real Income: What a Starting Physician Actually Earns

SSP eligibility note: SSP applies to primary care physicians — GPs, pediatricians, and FAP feldshers. Narrow specialists working in inpatient settings are not eligible. A graduate planning to pursue surgery, cardiology, or another hospital specialty should not count on the SSP supplement when modeling their income.
A contract student assigned to work in Kursk city with no SSP earns roughly the same as the guaranteed base package. A colleague in a rural district earns nearly three times as much, thanks entirely to federal funds. The financial case for accepting an urban placement is weak for a new graduate.
Housing Programs: What They Cover and What They Don’t
The rental compensation (Decree No. 419-pa) covers physicians working and living in rural settlements. Physicians placed in Zheleznogorsk or Kurchatov — both district-level cities — do not qualify. The program is valuable for rural placements but excludes a meaningful share of possible destinations.
Land plots under Law No. 33-ZKO are provided for individual housing construction. These plots frequently lack utility connections. A graduate with no savings and no construction experience cannot realistically act on this benefit within the first years of work.
Mortgage interest compensation (Decree No. 297-pa) requires a down payment and the ability to secure a mortgage. For most new graduates without accumulated capital, this benefit is deferred several years into the future.

A contract student placed in Kursk city with a net salary of ₽28,000 and no SSP would spend the majority of that income on rent. The arithmetic only works in smaller settlements where SSP applies.
Practical Training: Hidden Costs
The Ministry did not mention any reimbursement for travel or accommodation during internships. Students sent to districts outside their place of residence carry those costs themselves.

Over the full spetsialitet program, cumulative internship costs may reach ₽80,000–₽100,000. This sum does not appear in any official list of support measures.
Workplace Assignment and Contract Terms
The Ministry of Health of Kursk Oblast functions as the sponsoring organization for the entire regional contract quota at Kursk State Medical University. A student signing the contract agrees to work at any facility subordinate to the regional Ministry — the specific hospital or clinic is not specified at the time of signing.
Penalty-free termination under Government Decree No. 555 is restricted to four circumstances: recognition of Disability Group I or II for the contract student; the necessity of caring for a close relative with Disability Group I; relocation of a military spouse to a new duty station; and recognition of the contract student as a single parent. No other grounds exempt a graduate from repaying the cost of subsidized education.
Pros and Cons
Kursk Oblast offers contract students a functional but uneven package. The federal elements — SSP and Zemsky Doctor — carry the bulk of the financial value. The regional layer adds relatively little: no settling-in bonus, limited rental support, and housing tools that require capital most graduates don’t have on day one.
The main advantages are concentrated in rural placements. A physician working in a village or small town under 50,000 residents can reach a combined income of ~₽78,000 (~$780) per month from the first year, which is a competitive starting point for a new graduate. The tariff-based salary grid is transparent and verifiable — the ₽19,500 base figure is fixed by decree, so there is no ambiguity about the contractual minimum. The Zemsky Doctor payment of ₽1,000,000 (~$10,000) becomes accessible after the mandatory service period ends, representing a meaningful long-term incentive for those who complete the contract.
The disadvantages cluster around the city of Kursk and the early career period. A graduate placed in the regional capital receives no SSP, no rural rental compensation, and no land allocation — just the ~₽28,000 net base salary, which covers less than one month’s rent on a one-room apartment. The absence of any regional settling-in bonus means the financial gap between signing the contract and receiving the first full paycheck falls entirely on the student. The contract itself offers no certainty about placement location until graduation, which complicates personal planning for the full six years of study. Narrow specialists targeting inpatient hospital roles also lose access to SSP entirely.
The Kursk contract makes clear financial sense in a rural district. In the regional center, the same contract produces a starting income substantially below the market rate for educated professionals.
