Contract Medical Training in Zabaykalsky Krai: ₽2M Zemsky Doctor and ₽8,800/Month Subsidized Rent

The author sent official inquiries with eight core questions to every regional health authority in Russia. This is the response from the Ministry of Health of the Zabaykalsky Krai.
Part 1: What the Ministry of Health Told Me
Question 1: Zemsky Doctor Eligibility
Ministry’s response: Contract students (целевики) are eligible for the Zemsky Doctor program and are explicitly prioritized when the Ministry compiles its list of participating positions. The program runs through 2030.
What this means: The Ministry does not merely allow contract graduates to apply; it pre-reserves those positions for them. That is a meaningful difference from regions where contract students compete with open-market applicants for the same Zemsky Doctor slots.
Question 2: Settling-in Bonuses
Ministry’s response: One-time settling-in bonuses (подъёмные) are set by the internal bylaws of individual hospitals and depend on each organization’s finances. Details appear in the contract training proposals published on the «Work in Russia» portal (trudvsem.ru).
What this means: There is no regional-level guarantee. Whether a bonus exists, and how large it is, depends on the specific hospital posting you accept. Verify this in the actual offer during the admissions campaign.
Questions 3 and 4: Base Salary and Real Income
Ministry’s response: Salaries are governed by Decree of the Government of Zabaykalsky Krai No. 488 of September 26, 2024, and Ministry Order No. 23-P of October 16, 2024. The Order establishes base salaries and a set of supplements: 50% of the minimum wage for high-demand shortage positions, additional monthly payments for young specialists at 100%/60%/30% of the base salary in the first three years, and bonuses of 20–35% of the base salary for working in small settlements. Incentive payments are discretionary, not mandatory, and depend on available funds.
What this means: The pay structure contains real money beyond the base salary, especially in the first three years. The actual base salary figure is not specified in the letter; the Ministry points to Order 23-P, whose salary appendix is not publicly available. Real income has to be estimated from job postings.
Question 5: Housing
Ministry’s response: Five mechanisms are in place: 212 residential units purchased across the region’s districts; the Far Eastern Mortgage program; partial or full rental compensation from individual hospital budgets; service housing where municipal stock exists; and the «Affordable Rental Housing in the Far Eastern Federal District» program, which provides 250 apartments in Chita and 9 in the urban settlement of Zabaykalsk in 2025–2026.
What this means: The variety of options is genuine, not rhetorical. Two of the five are large federal programs with concrete numbers attached.
Question 6: Internship Support
Ministry’s response: Travel to internship sites and accommodation during internships are either specified in the individual contract training agreement or negotiated directly with the head of the relevant medical organization.
What this means: No regional-level guarantee exists. Check this clause in the specific contract offered to you on trudvsem.ru before signing.
Question 7: Choosing a Workplace
Ministry’s response: The Ministry distributes contract training quotas among employer hospitals before the admissions process opens. Each offer, including the specific workplace, is published on the «Work in Russia» portal.
What this means: This follows the federal procedure that took effect on May 1, 2024. You select a specific hospital vacancy, not a vague regional agreement, before you apply. The workplace is named before you commit.
Question 8: Contract Modification and Termination
Ministry’s response: Transfers within the region are possible by mutual agreement. Grounds for penalty-free termination are set exhaustively by Government Decree No. 555 of April 27, 2024; no additional grounds can be established locally.
What this means: Internal transfers exist as a genuine working mechanism. The federal termination list is fixed; the Ministry cannot expand it, and it cannot restrict it either.
Part 2: What I Found Through Independent Research
Zemsky Doctor Eligibility and Payment Amounts
The ₽2,000,000 (~$20,000) grant confirmed by the Ministry is correct for Zabaykalsky Krai. The region belongs to the Far Eastern Federal District (DFO), and all DFO regions qualify for the doubled Zemsky Doctor payment under Government Decree No. 1946. The standard payment in most of Russia is ₽1,000,000 (~$10,000).
The eligibility conditions are standard across all DFO regions: relocate to work in a rural settlement, a workers’ settlement, an urban-type settlement (пгт), or a town with a population under 50,000 residents; sign an employment contract; and remain in that location for at least five years.
The Ministry’s practice of pre-allocating Zemsky Doctor positions to contract graduates before the admissions cycle opens converts what is nominally an open competitive program into something close to a guaranteed payout for contract students who complete their training. A ₽2,000,000 lump sum on the first day of employment is the most direct financial incentive in the entire package.
Settling-in Bonuses
I checked the «Work in Russia» portal for current contract training offers from Zabaykalsky Krai medical institutions. At the time of this research, none of the publicly posted offers specified an explicit settling-in bonus. This confirms the Ministry’s framing: such payments are not a regional standard. Their presence depends on the individual hospital posting. Treat any bonus as a possibility to verify, not an assumption to rely on.
Base Salary and Real Income
Order 23-P is the governing document, but its salary appendix is not publicly accessible. To estimate actual starting pay, I reviewed physician job postings on HeadHunter and trudvsem.ru.
State medical institutions in Chita are advertising general practitioner positions, including roles open to candidates without experience, at ₽60,000 to ₽81,200 gross. The Chita Clinical Medical Center lists comparable ranges on trudvsem.ru. District hospital postings show similar figures.
These figures reflect the final advertised amount, which typically bundles several components together. To understand what a contract graduate would earn in a rural district hospital, where the supplement structure is more transparent, the calculation below models starting income using the components the Ministry described.

SSP — What the Ministry Did Not Mention
The regional Ministry of Health did not mention the Special Social Payment (SSP) in its letter. The program is federal, mandatory, and applies directly to the employment conditions most contract graduates in Zabaykalsky Krai will face.
SSP is a monthly supplement paid on top of the regular salary. It is tax-free and excluded from the average-earnings calculation used to determine benefits.
For a physician working in a settlement with a population under 50,000 (which describes most of Zabaykalsky Krai outside Chita), SSP amounts to ₽50,000/month. In settlements between 50,000 and 100,000 residents, the payment is ₽29,000/month. In Chita itself (population approximately 340,000), SSP does not apply.
SSP is available to primary care physicians: GPs, pediatricians, general practitioners, and FAP feldshers. Narrow specialists working in inpatient facilities do not receive it.

The gap between working in rural Zabaykalsky Krai and working in Chita is ₽50,000/month from SSP alone.
Regional Salary Context
According to Zabaykalkraistat, the average monthly nominal wage in Zabaykalsky Krai for early 2025 exceeded ₽90,000, reaching ₽99,140 in May 2025. A contract graduate working in a rural district hospital who qualifies for SSP would earn approximately ₽127,000, above the regional average from the first month of employment.
After the three- or five-year mandatory service period (отработка) ends, the young specialist supplement expires. Experience accumulates, qualification categories become accessible, and salary ranges shift upward. Vacancy data for physicians with three to six years of experience show cardiologists offered ₽100,000–140,000 net, general practitioners in corporate clinics (such as RZD-Medicine) at ₽120,000, and surgeons in outpatient clinics from ₽150,000 net.
Housing Programs
This is where Zabaykalsky Krai separates itself most clearly from the median Russian region.
The Far Eastern Mortgage offers an interest rate of up to 2% per annum. The maximum loan is ₽9,000,000 (~$90,000) for housing over 60 sqm, and ₽6,000,000 (~$60,000) otherwise. The minimum down payment is 20.1%. There is no age ceiling for medical workers employed in state or municipal institutions.
For context: a standard one-bedroom apartment in Chita costs approximately ₽5,300,000. In Krasnokamensk, the region’s second-largest city, one-bedroom prices range from ₽1,800,000 to ₽2,800,000. The ₽2,000,000 Zemsky Doctor grant covers the down payment with room to spare on a Krasnokamensk purchase, or covers the required 20.1% down payment on a Chita apartment and comes close to paying off the entire purchase in a smaller town.
The Affordable Rental Housing in the DFO program runs through 2025–2026 and provides 250 apartments in Chita and 9 in Zabaykalsk. In the first year, tenants pay one-third of the market rate; federal and regional budgets cover the rest. According to media reports, rent under this program for an apartment of approximately 37 sqm comes to around ₽8,800/month.
Beyond these two federal programs, the Ministry confirmed three additional mechanisms: 212 residential units already purchased and distributed across the region’s districts; partial or full rental compensation through individual hospital bylaws; and service housing in municipalities where public stock is available.

The subsidized rental rate in Chita saves a physician up to ₽30,000/month compared to market rent for a comparable apartment.
Internship Support
No regional regulation guaranteeing travel costs or accommodation for all contract students during internships was found in public sources. Check this clause in the specific contract training agreement posted on trudvsem.ru before signing. The Ministry’s position is that outcomes are resolved individually by hospital, which means they vary.
Workplace Selection
Zabaykalsky Krai follows the federal system that came into force on May 1, 2024. All contract training offers, including the designated employer hospital and location, must be published on the «Work in Russia» portal before the admissions cycle opens. An applicant selects a specific vacancy at a specific institution. The sponsoring organization (заказчик) in Zabaykalsky Krai is the regional Ministry of Health, which designates individual hospitals as the actual workplaces. The name of that hospital is visible before you sign.
Contract Modification and Termination
Transfers within the region are possible by mutual agreement between the graduate and the Ministry. The Ministry indicated it actively considers such requests.
Penalty-free termination is governed by Section V of Government Decree No. 555 of April 27, 2024. The grounds are fixed and exhaustive. They include medical contraindications (recognized by a medical commission) to living in the designated location, affecting the student, their spouse, or their child; recognition of a parent, spouse, or child as a Group I disabled person when the contract work prevents the student from providing the required care and no other legally obligated caregiver exists; the student being the sole parent of a minor child; and the student’s spouse being a career military officer transferred to a new posting where the student cannot be employed.
No other grounds may be added by local agreement. The list protects against extreme personal circumstances but does not allow exit for reasons of preference or career change.
Pros and Cons
Zabaykalsky Krai’s contract training program is built around three elements that reinforce each other: the ₽2,000,000 (~$20,000) Zemsky Doctor grant, the 2% Far Eastern Mortgage, and SSP payments of up to ₽50,000/month. All three apply specifically because the region belongs to the DFO — conditions most of Russia’s regions cannot offer.
The advantages are concrete. From the first month of employment in a rural district, a contract graduate’s combined income (net salary plus SSP) can exceed ₽127,000, above the regional average of ₽99,140. The Zemsky Doctor payout arrives as a lump sum and covers the full down payment on a Far Eastern Mortgage. The subsidized rental program cuts housing costs to ₽8,800/month in Chita, roughly one-quarter of the market rate. Workplace transparency is guaranteed by the federal posting system, with the specific hospital named before the applicant commits. The Ministry pre-allocates Zemsky Doctor positions for contract graduates, removing the competitive uncertainty present in other regions.
The disadvantages are equally concrete. The mandatory service period commits you to the region for three to five years. Settling-in bonuses are not guaranteed at the regional level and depend on the finances of whichever hospital you join. Internship travel and accommodation costs are not covered by default. The young specialist supplement disappears after three years, reducing take-home pay before experience-based raises compensate. Chita, as a city above 100,000 residents, does not qualify for SSP, so a doctor choosing to work in the regional capital accepts ₽50,000/month less than a counterpart in a district hospital.
The decision to sign a contract training agreement is a multi-year commitment that determines where you live and work during the first years of your career. The support available in Zabaykalsky Krai is real and documented. So are the obligations.
