Contract Medical Training in the Urals: Which District Pays ₽3.5M to a Starting Physician


This article is part of the Navigator for Contract Students project — a systematic investigation of contract training agreements across Russia’s 85 regions. For Sverdlovsk Region, I applied the same eight-question framework used across every regional study: Zemsky Doctor eligibility, financial incentives, real salaries, housing programs, internship costs, workplace selection, and contract modification rules.

Note: As of 2025, 1 USD ≈ 100 RUB. All figures are in Russian rubles (₽) unless otherwise stated.


Part 1: What the Ministry of Health Told Me

Question 1: Zemsky Doctor / Zemsky Feldsher

Ministry’s response: Medical workers who relocate to rural settlements, urban-type settlements, or towns with a population under 50,000 are eligible for the Zemsky Doctor one-time grant. For physicians, the payment is ₽1,000,000 (~$10,000) in standard rural areas and ₽1,500,000 (~$15,000) in remote territories. Feldshers receive ₽500,000 (~$5,000) and ₽750,000 (~$7,500) respectively. The program runs until 2030 under Government Decree No. 1640 of December 26, 2017. Service obligations begin on the date the grant contract is signed and amount to five years. With Ministry approval, a one-time change of workplace is permitted without returning the grant.

What this means: Contract students (целевики) can participate in the Zemsky Doctor program after graduating. The Ministry did not specify which districts qualify as «remote territories» for the higher rate. Based on regional regulations, Shalinsk Municipal District is one confirmed example. Sverdlovsk Region is not part of the Far North, equivalent territories, or the Far Eastern Federal District (DFO). The ₽2,000,000 (~$20,000) federal rate does not apply here.


Question 2: Settling-in Bonus (подъёмные)

Ministry’s response: Graduates who start work in a state medical organization in the year of graduation receive a settling-in bonus (подъёмные) under Law of the Sverdlovsk Region No. 91-OZ of November 21, 2012. The amount is ₽100,000 (~$1,000) in urban facilities and ₽150,000 (~$1,500) in rural facilities. The employment contract must be open-ended or for at least three years. The mandatory service period (отработка) is three years. Changing workplaces while retaining this bonus is not permitted under current legislation.

What this means: The «year of graduation» condition is a hard cutoff. A gap year, military service, or any delay in employment forfeits the right to the bonus permanently. The Ministry’s letter made no mention of the federal Special Social Payment (SSP). That omission is addressed in Part 2.


Question 3: Base Salary

Ministry’s response: Salaries in state health institutions are governed by Government Decree of the Sverdlovsk Region No. 866-PP of September 30, 2015. Total compensation consists of a base salary, compensatory payments, and incentive payments. The figures in the Model Regulation represent minimums; the facility director may raise them based on qualification requirements and available funding. The base salary must constitute at least 50% of total compensation.

What this means: The Ministry cited the document without stating the actual minimum salary figure. The appendix to Decree No. 866-PP (as amended April 4, 2024) shows a minimum base salary of ₽22,128 (~$221) per month for a starting physician specialist. That figure only becomes meaningful when combined with rural supplements and incentive payments.


Question 4: Real Income

Ministry’s response: The Ministry referenced Presidential Decree No. 597 of May 7, 2012, which requires physician salaries to reach 200% of the regional average. It noted that salary targets are set for all regional health institutions and are adjusted in line with growth in projected labor income across the region.

What this means: The 200% target is a system-wide benchmark, not a personal guarantee. New graduates, who typically start on shorter working-time norms and without seniority supplements, commonly earn below the statistical average. The Ministry provided no data on actual starting salaries.


Question 5: Housing Programs

Ministry’s response: Under the state program approved by Decree No. 1296-PP of October 24, 2013, physicians and nursing staff working in settlements with up to 100,000 residents can receive social payments from the regional budget for purchasing or constructing a home. Local governments provide service housing, social and commercial lease agreements, and dormitory places from the municipal housing fund. Medical workers can receive free land plots for individual housing construction under Law No. 18-OZ of July 7, 2004. Workers in rural settlements and urban-type settlements receive utility cost compensation under Decree No. 690-PP. Family Mortgage rates start at 6%; the Sverdlovsk Housing Mortgage Agency (SAIZK) offers rates from 0.1%.

What this means: Support options exist at several levels: regional, municipal, and federal. Conditions vary substantially across municipalities. Family mortgage and SAIZK programs are tied to family eligibility criteria, not universal entitlements for all medical workers.


Question 6: Clinical Rotations

Ministry’s response: Support measures during practical training can be provided by the sponsoring organization (заказчик). No specific travel or accommodation guarantees were stated.

What this means: Travel and accommodation costs during rotations are not guaranteed. For students studying in Yekaterinburg and assigned to a facility in the north of the region (400–530 km away), cumulative costs over six years can exceed ₽100,000.


Question 7: Choosing a Workplace

Ministry’s response: Contract training is organized under Government Decree No. 555 of April 27, 2024. The sponsoring organization can be identified at any stage of the contract training agreement (целевой договор). Support measures may be set by the employer, including during practical training.

What this means: As of May 1, 2024, applicants select offers through the «Work in Russia» portal (trudvsem.ru). If the sponsoring organization listed is a specific named hospital, the workplace is known at signing. If the sponsoring organization is the regional Ministry of Health, the actual facility may be assigned at a later stage.


Question 8: Contract Terms and Modification

Ministry’s response: Contract training is governed by Government Decree No. 555. The Ministry did not detail grounds for termination or the mechanics of workplace transfer.

What this means: Federal rules under PP RF No. 555 set the grounds for penalty-free termination. The regional Zemsky Doctor transfer rules are established by Decree No. 250-PP of April 6, 2023. Part 2 below covers the regulatory conflict that arises when a transfer occurs.


Part 2: What I Found Through Independent Research

Zemsky Doctor: Payment Amounts and Eligible Districts

The standard Zemsky Doctor payment in Sverdlovsk Region is ₽1,000,000 for physicians and ₽500,000 for feldshers in qualifying rural areas and towns under 50,000. For designated remote and hard-to-reach territories, the amounts rise to ₽1,500,000 for physicians and ₽750,000 for feldshers. Shalinsk Municipal District is one confirmed example at the higher rate; the full list is set through regional Decree No. 250-PP of April 6, 2023.

Since July 2024, under Government Decree No. 954, Zemsky Doctor participants may change their workplace once within the region without returning the grant, provided the new position is equivalent and the new facility meets program requirements.

Special Social Payment (SSP): What the Ministry Omitted

The Ministry’s letter did not mention the federal Special Social Payment (ССВ), governed by Government Decree No. 2568. SSP is a monthly supplement paid to primary care physicians: district GPs, pediatricians, and FAP (rural medical outpost) feldshers. Narrow specialists working in inpatient facilities do not receive it. The payment is tax-free.

Settling-in Bonus: The «Year of Graduation» Rule

The regional settling-in bonus requires employment to begin in the year of graduation. A gap for any reason (military service, illness, delayed contract) cancels entitlement regardless of circumstance. This applies to both the ₽100,000 city rate and the ₽150,000 rural rate.

Base Salary: What the Decree Actually Says

The salary structure is regulated by Decree No. 866-PP of September 30, 2015 (as amended April 4, 2024, by Decree No. 227-PP). A mandatory rule introduced in 2024 requires the base salary to constitute at least 50% of total compensation for medical staff.

Rural supplement: workers in rural settlements and urban-type settlements receive a 25% increase on their base salary (Clause 6 of the Model Regulation). A second-level physician specialist in a rural setting: ₽22,128 × 1.25 = ₽27,660 (~$277). With the 50% rule, when the base salary is ₽22,128, total compensation must reach at least approximately ₽44,000 (~$440).

Real Income: What Physicians Actually Earn

According to Sverdlovskstat, the average wage across the region was ₽78,493 (~$785) for all of 2024 and ₽87,158 (~$872) in the first half of 2025. Rosstat and job vacancy data put average physician salaries at ₽74,000–₽80,000 (~$740–$800) per month.

Municipal Bonus Programs: The Northern Districts

Several northern municipalities operate additional payment programs that far exceed the regional baseline, creating direct financial competition for physicians.

A physician choosing Ivdel or Volchansk can combine: ₽1,500,000 (Zemsky Doctor, remote territory rate) + ₽1,500,000–₽2,000,000 (municipal bonus) + ₽150,000 (regional settling-in bonus) = over ₽3,500,000 (~$35,000) at the start. Current apartment prices in those towns run ₽1,500,000–₽2,500,000 (~$15,000–$25,000), making outright purchase without a mortgage a realistic scenario.

Talitsa Municipal District operates a different model: instead of a lump sum, the municipality subsidizes mortgage payments. Under Decree No. 710 of December 29, 2017, the program covers 50% of the down payment (up to ₽350,000, ~$3,500) and 50% of monthly mortgage installments.

Housing Programs

Under the state program approved by Decree No. 1296-PP, budget-sector workers, including medical staff, can receive a social payment covering 20% of the calculated housing cost after at least three years of employment. The procedure is governed by Decree No. 54-PP of February 1, 2024.

Municipal governments provide service housing and social lease agreements; specific conditions vary by municipality. Land plots for individual housing construction are available free of charge under Law No. 18-OZ. Workers in rural settlements and urban-type settlements receive utility cost compensation under Decree No. 690-PP.

Internship Costs

The Ministry confirmed that practical training support depends on the employer’s decision, with no regional guarantee. For students studying in Yekaterinburg and assigned to a northern facility, each rotation involves real out-of-pocket costs.

Choosing a Workplace

Offers are published on the «Work in Russia» portal (trudvsem.ru). From the start of the 2024 application cycle, applicants select from listed offers placed by sponsoring organizations. Examining each offer carefully is necessary: a specific named hospital means the workplace is defined at signing; the regional Ministry of Health listed as the sponsoring organization means the actual facility may be assigned at a later stage.

Contract Modification and Termination

Workplace transfer: With Ministry approval, a Zemsky Doctor participant may change their place of work once, retaining the grant in full. The new position must be equivalent, the new facility must meet program requirements (rural area or town under 50,000), and the move must remain within the region. The five-year service obligation continues from the original signing date.

A regulatory conflict: Changing workplaces triggers a clash between two frameworks. Under Zemsky Doctor rules (Decree No. 250-PP), the transfer is permitted and the grant is retained. Under the regional settling-in bonus rules (Law No. 91-OZ), changing employer before completing the three-year mandatory service period triggers repayment. The physician keeps the Zemsky Doctor grant but must return the ₽150,000 settling-in bonus.

Penalty-free termination: Under Government Decree No. 555, the contract training agreement can be dissolved without financial penalties in four situations: assignment of a Group I or II disability to the physician; becoming the sole caregiver for a close relative with a Group I disability; the spouse’s transfer to a new military posting; declaration of an emergency in the region. Unilateral withdrawal without a qualifying ground requires repayment of received grants in proportion to the unserved portion of the mandatory service period.


Pros and Cons

Sverdlovsk Region operates one of the more fully articulated rural incentive systems in the Ural Federal District, with federal, regional, and municipal layers that can be stacked. The arithmetic is real: a physician prepared to work in Ivdel or Volchansk can receive more than ₽3,500,000 (~$35,000) at the start of their career, enough to purchase an apartment in those towns outright.

The advantages are most concentrated in smaller northern municipalities. The Zemsky Doctor program provides ₽1,000,000–₽1,500,000 depending on territory classification, and since 2024 the single permitted workplace transfer is formalized in regional regulations, a protection found in relatively few regions. SSP adds ₽50,000 per month for primary care physicians in towns under 50,000 residents, pushing combined monthly income past ₽115,000. Four northern municipalities (Ivdel, Volchansk, Severouralsk, Kachkanar) offer substantial additional bonuses, while Talitsa operates a mortgage subsidy program for those preferring gradual housing support over a lump sum. The Zemsky Doctor program extends to 2030, giving applicants entering in 2025 a confirmed planning horizon. Free land plots, service housing from municipal funds, and utility compensation for rural workers round out the package.

The disadvantages require equal attention. The «year of graduation» rule on the settling-in bonus is unforgiving: any gap between finishing studies and starting work cancels the payment, even for reasons outside the graduate’s control. A permitted workplace transfer under Zemsky Doctor rules still triggers repayment of the ₽150,000 settling-in bonus under a separate regional law. The conflict is not resolved by the transfer approval. Sverdlovsk Region does not qualify for the ₽2,000,000 federal Zemsky Doctor rate reserved for the Far North, equivalent territories, DFO, and Arctic zone. Internship travel costs are not guaranteed and accumulate across six years of training. Base salaries are low in absolute terms: a starting specialist earns ₽22,128 in base pay, and everything above that depends on incentive payments at the discretion of the facility director. SSP and housing subsidies do not apply in cities over 100,000 residents, ruling out Yekaterinburg and Nizhny Tagil.

The decision to sign such a contract should be made with a clear picture of both the financial opportunity and the five-year service commitment that comes attached to it.
