Contract Training for Doctors in Dagestan: Free Land After 5 Years and ₽110,000/Month With SSP

The Republic of Dagestan is the most populous region in the North Caucasus, with over 3.2 million residents. The republic is actively developing its healthcare system and recruiting young physicians through contract training. What makes Dagestan stand out is a specific regional instrument found almost nowhere else: free land grants that convert into full ownership after five years of service.
Analysis of Official Response
At the time of publication, no official response has been received from the Ministry of Health of the Republic of Dagestan. The analysis below is based on data from open official sources.
Independent Research
Question 1. Zemsky Doctor and Zemsky Feldsher Programs
Dagestan does not qualify as a Far North region or a territory equated to the Far North under Government Decree No. 1946. Accordingly, the applicable Zemsky Doctor payment rates are:
Physicians in standard rural areas receive ₽1,000,000 (~$10,000). Physicians in remote and hard-to-reach territories receive ₽1,500,000 (~$15,000). Feldshers in standard rural areas receive ₽500,000 (~$5,000), and feldshers in hard-to-reach areas receive ₽750,000 (~$7,500).
According to the Dagestani Ministry of Health, the 2024 plan was to recruit 170 medical workers: 149 physicians and 21 mid-level staff. By year-end, 147 physicians and 30 mid-level staff had been placed. For 2025, approved quotas stand at 120 physicians and 15 mid-level specialists; projections for 2026 suggest an increase to 150 physicians.
Contract students (целевики) are eligible for the Zemsky Doctor program, but only at facilities where staffing falls below 60%. Eligibility arises after the graduate has fulfilled the primary obligations of the contract training agreement (целевой договор).
Question 2. Settling-in Bonuses and Federal SSP
No regional one-time settling-in bonuses (подъёмные) beyond the Zemsky programs were found in open sources from the Ministry of Health.
The federal Special Social Payment (SSP), established under Government Decree No. 2568, receives little attention from regional health ministries, yet it can nearly double a rural physician’s income. The current rates are shown in Table 1.

SSP is available to district therapists, pediatricians, general practitioners, and FAP (rural medical outpost) feldshers. Narrow specialists working in inpatient hospital departments do not receive it.
Regional Support: Land Plots and Utility Subsidies
Republic of Dagestan Law No. 116-Z (dated December 29, 2017) grants medical workers under contract training agreements or Zemsky programs the right to use a land plot free of charge. The mechanism has two stages. During the first five years, the specialist may use the land, build a house, and farm it, but cannot sell the plot. After five years of continuous service, the right of use converts into full ownership at no cost.
The retention logic here is blunt: a physician who resigns in year four loses a plot they spent four years earning. In a republic with high birth rates and deep cultural attachment to property, this instrument carries real weight.
Government Decree of the Republic of Dagestan No. 409 (dated November 25, 2022) provides utility compensation (heating and electricity) for rural medical workers. In regions where private home heating is a material household expense, this directly increases disposable income.
Question 3. Base Salary
The exact minimum base salary for physicians in Dagestan is not publicly specified in open sources. As of January 1, 2025, the regional minimum wage (MROT) is ₽22,440. In certain mountain districts, regional multipliers of 1.1-1.3 push the minimum to ₽24,684-₽29,172.
In August 2025, the Dagestan Medical Workers’ Union and the Ministry of Health signed an Industry Agreement for 2025-2028, fixing specific pay conditions, bonuses, additional leave entitlements, and working hour norms for the period ahead.
Question 4. Real Income
Rosstat data for March 2025 puts the average wage in Dagestan at ₽47,904, one of the lowest figures in Russia. Job board postings tell a different story for physicians specifically.

A district therapist at a rural facility in a town under 50,000 residents earns a base salary of approximately ₽60,000. Adding the federal SSP of ₽50,000/month produces a combined monthly income of ₽110,000 (~$1,100). That is 2.3 times the regional average of ₽47,904 and, in many cases, exceeds what private clinics in Makhachkala advertise.
Question 5. Housing Programs
Beyond the land grant program (Law No. 116-Z) and utility subsidies (Decree No. 409), no additional regional housing programs (rent subsidies, preferential mortgages, or service housing) were identified in open sources.
The rental market in Dagestan remains affordable. A one-bedroom apartment in Makhachkala starts at ₽12,000-₽16,000/month on a long-term lease; a well-maintained unit averages ₽20,000-₽30,000. Derbent, the republic’s second city, is cheaper.

At a combined income of ₽110,000/month, rent in Makhachkala consumes 15-30% of take-home pay, a ratio most financial planners would consider comfortable.
Question 6. Internship Support
No travel or accommodation compensation for contract students during practical placements was found in open sources. In practice, these terms are negotiated individually within each contract training agreement.
For a student based in Moscow, one round-trip to Makhachkala costs approximately ₽5,000-₽10,000 in flights alone. A 28-day placement, including accommodation, runs ₽25,000-₽35,000 per trip. Over four mandatory practicums, the total out-of-pocket cost can exceed ₽100,000.
On the logistical side, Makhachkala’s Uytash Airport offers direct routes to Moscow with major carriers: Aeroflot, Pobeda, S7. The flight takes about three hours. Ticket prices in low season can drop to ₽3,700-₽5,000 one way, often cheaper than a reserved-seat train berth. Students choosing Dagestan retain reasonable access to Moscow and its medical infrastructure within a single travel day.
Question 7. Choosing a Workplace
Since May 1, 2024, the federal procedure under Government Decree No. 555 requires prospective contract students to select offers through the «Work in Russia» portal (trudvsem.ru). When reviewing listings, the identity of the sponsoring organization (заказчик) matters: if the sponsor is a named medical facility, the workplace is set from day one. If the sponsor is the regional Ministry of Health, the specific assignment may be determined after graduation.
Question 8. Contract Terms and Termination
Workplace transfer. Since July 2024, participants in the Zemsky Doctor and Zemsky Feldsher programs may transfer to a different facility once during the contract period (Government Decree No. 954 of July 13, 2024). The new facility must be in a rural area or small town within the same region.
Penalty-free termination. Federal law (Government Decree No. 555) defines grounds for exiting the contract without financial liability. These include establishment of a Category I or II disability, the need to care for a first-degree relative with a Category I disability, relocation following a spouse’s military transfer to a new posting, recognition as the sole parent of a child under 18, and other circumstances specified by law.
Pros and Cons
Dagestan is a region with low living costs, real financial upside from federal SSP, and one regional instrument — the land grant — that sets it apart from most other republics. The picture is not without friction.
The program’s advantages are tangible. Rent from ₽12,000/month and accessible prices for groceries and services allow a physician to save a meaningful share of income. A district doctor in a small town earns ₽110,000/month after adding SSP, which is 2.3 times the regional average, without working multiple jobs or taking private patients. The land grant under Law No. 116-Z is a structurally rare offer: free land transitioning to full ownership after five years. In a republic where property ownership carries cultural and practical importance, that is not a trivial incentive. Utility compensation under Decree No. 409 further trims household costs. The Zemsky Doctor program is operational and placing physicians at scale: 147 doctors recruited in 2024, with 150 projected for 2026. Infrastructure investments are real, with 20 rural clinics and FAPs built in 2024 and 75 facilities renovated. Air access to Moscow at ₽3,700-₽5,000 keeps the capital within a three-hour reach.
The disadvantages are worth naming directly. Base salaries in government facilities start around ₽50,000, below the national average for physicians. The Zemsky Doctor payment is available to contract graduates only at facilities with staffing below 60%, which by definition means understaffed, high-workload environments. The land plot is not a house: a young specialist still needs capital for construction. Mountain districts present genuine geographic isolation, with limited access to professional development, specialist consultations, and urban infrastructure. Narrow specialists and physicians at facilities serving above 100,000 residents receive no SSP. Remote mountain areas carry logistical challenges that rural postings in flatter regions do not.
The five-year land vesting schedule rewards those who stay, and penalizes those who leave early. That is a deliberate design choice. Anyone considering Dagestan should run the numbers on their specific district, the specific facility, and the specific contract conditions before signing.
