Contract Training in Buryatia: ₽2M Zemsky Doctor, Full Mortgage Coverage — But 8–10 Year Commitment

The dream of becoming a doctor led me to explore contract training (целевое обучение) as one of the most realistic paths to achieving this goal. However, I quickly realized that a contract training agreement (целевой договор) is not just an admission perk—it’s a serious contract defining your career and life for the next 8–10 years.
Faced with a lack of complete and honest information, I decided to act. I formulated the eight core questions every prospective contract student should ask—grouped into three topics: finances, housing, and contract conditions—and sent them to the Ministry of Health of the Republic of Buryatia. The goal was simple: to obtain official clarifications in order to make an informed decision.
Recently, I received an official reply. It provided valuable insights but left significant gaps. This article is divided into two parts. First, I analyze exactly what the Ministry answered. Then, I share the results of my own independent research—filling in the questions they ignored and supplementing those they only partially addressed.
Part 1: What the Ministry of Health Told Me
I asked eight questions. The Ministry addressed most of them, though with varying levels of detail. Below is my analysis of their responses.
Question 1: Do the Zemsky Doctor Program and Regional Payments Apply to Contract Students?
Ministry’s response: The letter clearly states that lump-sum compensatory payments under the Zemsky Doctor program and regional programs are provided only to medical workers who do not have unfulfilled obligations under contract training agreements. The program is valid until 2030, and its extension is decided at the federal level.
What this means: First, you must complete your mandatory service period (отработка)—usually 3–5 years. Only after that can you apply for Zemsky Doctor payments, which in turn require another 5 years of service. For contract students aiming for this payout, the total commitment could indeed reach 8–10 years.
Question 2: Are There Settling-in Bonuses Upon Employment?
Ministry’s response: No direct answer. The letter states that «social support measures and payments to young specialists upon employment are provided in accordance with the local regulations of the employer.»
What this means: The Ministry redirects this question to individual hospitals. Settling-in bonuses (подъёмные) are not guaranteed at the regional level and must be clarified with each medical organization separately during contract negotiations.
Question 3: What Is the Exact Base Salary?
Ministry’s response: The minimum base salary (должностной оклад) for a physician in the Republic of Buryatia is ₽22,102. The letter notes that final remuneration is calculated according to the regulations in effect at the time of employment. The specific normative act establishing this figure was not cited.
What this means: A specific number is provided, but without a reference to the regulatory document. More importantly, the lack of information about the salary structure—regional multipliers, incentive payments, and supplements—makes this figure nearly meaningless for understanding real income.
Question 4: What Are the Guaranteed Monthly Allowances and Real Income?
Ministry’s response: No direct answer. As with settling-in bonuses, the topic of allowances and real income falls under «local regulations of the employer.»
What this means: The Ministry does not provide generalized data on the actual salaries of young specialists. This critical information—what you’ll actually earn—must be researched independently.
Question 5: What Housing Programs Exist and What Are Their Conditions?
Ministry’s response: This was the most detailed section of the letter. The Ministry listed several programs:
- Rent compensation: Mentioned as a measure, but without specific amounts.
- Full mortgage compensation: Under Government Decree No. 222 (dated May 19, 2021), medical workers receive a monthly payment covering the full principal and interest for loans taken under the «Rural Mortgage» and «Far Eastern Mortgage» programs. Conditions: term up to 15 years, 20% down payment, age up to 65, employment in a subordinate organization.
- Free land plots: Under Law No. 601-III «On Land» (dated December 30, 2003), medical workers are given land in state or municipal ownership for individual housing construction or farming.
- Service housing construction: In 2024, construction began on 64 apartments in 8 two-story buildings across 7 districts and the city of Kyakhta.
The catch: The letter contains a glaring contradiction. The mortgage compensation section states a maximum property value of «no more than 6,000,000 (two million five hundred thousand) rubles.» The numerical value (6 million) and the written value (2.5 million) do not match—this requires clarification.
Question 6: Is Support Provided for Internships Away from the University?
Ministry’s response: The answer mirrors the one for settling-in bonuses: support for contract students «during practical training is provided in accordance with the local regulations of the employer.»
What this means: This question must be addressed directly with the future sponsoring organization (заказчик) when signing the contract. No regional-level guarantees exist.
Question 7: How Does the Workplace Selection Mechanism Work?
Ministry’s response: The procedure is straightforward. Citizens apply to a subordinate medical organization between April 1 and June 10. After this period, they can contact any medical organization from the published list that has vacancies for contract training agreements.
What this means: The applicant chooses a specific hospital from the Ministry’s published list (46 institutions for the 2025–2026 academic year) and concludes a contract directly with that facility—not with the Ministry as a whole. You know your exact workplace before signing.
Question 8: Can the Contract Be Modified or Terminated Early?
Ministry’s response: Any changes to the contract, including replacing the employer organization or early termination, are regulated by Federal Government Decree No. 555 (the letter incorrectly cites it as dated «April 27, 2025″—it should be 2024). Specific grounds for termination were not listed.
What this means: The Ministry references the federal law but provides no practical details. The actual grounds for penalty-free termination require independent investigation.
Part 2: What I Found Through Independent Research
The official response provided a foundation but left critical gaps. To fill them, I conducted my own investigation using open sources—government decrees, job portals, real estate platforms, and statistical agencies.
Zemsky Doctor Eligibility and Amounts
The Ministry’s answer was clear: complete your service, then apply. My research confirms this. The Zemsky Doctor program runs until 2030. For physicians moving to rural settlements in the Far Eastern Federal District (which includes Buryatia), the payout is ₽2,000,000 (~$20,000). In small towns under 50,000 residents, the amount is ₽1,000,000 (~$10,000). The mandatory condition is a 5-year commitment.
For contract students, the math is stark: 3–5 years to fulfill the contract training agreement, then another 5 years for Zemsky Doctor. Total: 8–10 years of binding obligations.
Settling-in Bonuses and Regional Payments
As the Ministry indicated, this is regulated by individual hospitals. However, Buryatia offers additional regional lump-sum payments:
- ₽1,000,000 (~$10,000) for physicians moving to cities with populations under 50,000 (Gusinoozersk, Kyakhta, Zakamensk, Babushkin).
- ₽1,000,000 (~$10,000) for physicians in the most scarce specialties (anesthesiologist-resuscitator, obstetrician-gynecologist, surgeon, traumatologist-orthopedist) employed under the Zemsky Doctor program.
These payments are separate from the federal Zemsky Doctor program and can be combined under certain conditions.
Base Salary and the Actual Regulatory Document
The Ministry cited a base salary of ₽22,102. This is established by the Model Regulations on the Remuneration System for State Medical Institution Employees, approved by Ministry of Health Order No. 420-OD (dated July 12, 2019, with amendments through April 8, 2024). This document regulates the entire payment structure for public healthcare workers in Buryatia.
Real Income—What You’ll Actually Earn
This is the most critical question. To understand whether a young physician will feel financially secure, I compared potential income with the regional average. According to Rosstat, the average nominal wage in the Republic of Buryatia in 2024 was ₽72,823.
A physician’s real salary in Buryatia consists of several components:
- Base salary: ₽22,102
- Incentive and compensatory payments: seniority bonuses, professional category supplements, hazard pay—set by each hospital
- Regional multiplier: 1.2 in Ulan-Ude (+20%), 1.3 in northern districts (+30%)
- Northern bonus: In Ulan-Ude, 10% after the first year, increasing by 10% every two years, up to 30%
- Special Social Payment (SSP): This is a key factor often omitted by regional ministries. Since March 2024:
Towns under 50,000 residents: ₽50,000/month for physicians
Cities 50,000–100,000 residents: ₽29,000/month for physicians
Cities over 100,000 (Ulan-Ude): ₽14,500–18,500/month

Bottom line: A young specialist’s starting income in Ulan-Ude will likely range from ₽60,000–70,000 net (~$600–700), comparable to the regional average. In a district hospital, factoring in the maximum SSP of ₽50,000, total monthly income can easily exceed ₽100,000 (~$1,000)—significantly above the average.
Career trajectory after 5 years: I analyzed vacancies for physicians with 3–6 years of experience. For example, a General Practitioner role in Ulan-Ude lists a salary from ₽129,250 (before tax). Other high-paying vacancies (₽130,000–140,000) suggest that after completing the mandatory service period, physicians can claim incomes placing them among the region’s top earners.
Housing Programs—The Details
Rent compensation: The amounts remain unknown, but market analysis shows this is critical. Buying immediately is difficult—you need somewhere to live from day one. I analyzed rental prices on CIAN, Avito, and Domklik.

Service housing: The 64 apartments under construction will be furnished and finished—a significant advantage.
Full mortgage compensation: The contradiction (6 million vs. 2.5 million) is resolved by examining the «100 Houses for Doctors» program—a joint initiative by the Ministry and Sberbank implementing Decree No. 222. The conditions specify a maximum property value of ₽2,500,000 (~$25,000). This is likely the limit for full compensation. The mortgage itself is issued under the federal «Far Eastern Mortgage» program (up to 2% interest, ₽6–9 million maximum) or «Rural Mortgage.»
Land plots: While Law No. 601-III «On Land» mentions free land for medical workers, the specific provision is difficult to locate. However, the official letter’s reference is promising and worth investigating with local authorities.
Internship Support
As expected, no centralized information exists. This must be negotiated directly with the hospital’s HR department when signing the contract.
The Distribution Mechanism
The Ministry’s description is accurate. The procedure is confirmed on the official Ministry of Health website. Between April 1 and June 10, applicants choose a specific medical organization from the published list and submit documents to its HR department. The 2025–2026 list includes 46 institutions across the republic—from central district hospitals to republic-level dispensaries.
Contract Termination—Your Legal Safety Net
The Ministry cited Federal Decree No. 555 but made a typo. The correct document is dated April 27, 2024. Section V details grounds for suspension or termination without penalties:
- Medical contraindications for the student or close relatives (spouse, children, parents) to work or reside in that location
- Disability status (Group I or II)
- If the student is the spouse of a military servicemember or government employee transferred to another location
- Single parenting of a child under 3 years old
- If the employer ceases operations or cannot provide the agreed workplace
This federal law serves as your legal «safety net» if circumstances change.
Pros and Cons
Contract training in Buryatia is a serious, well-structured program offering a unique support package, especially regarding housing. However, it demands long-term commitment (potentially 8–10 years) and willingness to work in rural areas or small towns, where maximum financial incentives are concentrated.
The program’s advantages include substantial financial support: Zemsky Doctor payments up to ₽2,000,000 (~$20,000), regional bonuses up to ₽1,000,000 (~$10,000), and full mortgage compensation for properties up to ₽2,500,000 (~$25,000). SSP significantly boosts rural income — in towns under 50,000 residents, the ₽50,000/month SSP can push total income over ₽100,000, well above regional averages. The republic provides free land plots for housing construction or farming, and 64 furnished apartments are under construction across 7 districts. You know your exact employer before signing, and federal law provides multiple grounds for penalty-free contract termination if circumstances change.
The disadvantages center on commitment length and uncertainty. Accessing Zemsky Doctor requires 8–10 years total (3–5 years for the contract plus 5 years for Zemsky), which represents a significant portion of your early career. Settling-in bonuses are not guaranteed at the regional level and must be negotiated individually with each hospital. Housing support details remain unclear — rent compensation amounts are not specified, and the mortgage program has contradictory documentation (₽2.5M vs. ₽6M limit). No regional support exists for travel and accommodation during mandatory clinical rotations, creating hidden internship costs. The base salary alone is low at ₽22,102, meaning little without understanding the full supplement structure. Finally, maximum benefits require working in small towns or villages, which limits urban career options and may not align with long-term professional goals.
The decision to sign such a contract must be deliberate. This investigation aims to help you make an informed choice.
